Description
THIS REQUIREMENT IS PROVIDED FOR IN THE INSTALLATION'S FY09 EXECUTION PLAN, PENDING CONGRESSIONAL APPROVAL OF THE FY09 APPROPRIATION'S BILL. ALL PRICING FROM OFFERS RECEIVED SHALL REMAIN VALID THRU OCT 31, 2008. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number FA4625-08-Q-0021 is issued as a request for quotation to service Exhaust Hoods and Ducts at Whiteman AFB, MO. The acquisition performance period is for one (1) 12-month base period and four (4) 12-month option periods. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-25, DFARS DCN 20080513, AFFARS AFAC 2008-0128. The NAICS code is 561790 with the corresponding small business size standard of $6.5 million. This acquisition is set aside 100% for small business concerns. General Wage Decision # 86-0497 Revision 28 dated 1/17/2008 applies to this acquisition and can be found at http://www.wdol.gov/. All offers are to remain valid until 31 Oct. 2008. Basis for Award; A FFP contract shall be awarded to the lowest priced, technically acceptable offer. Paper copies of the Request for Quotation shall not be made available. FY09 STATEMENT OF WORK FY09 KITCHEN HOODS AND EXHAUST SYSTEMS MAINTENANCE AND CLEANING SERVICES SCOPE: The contractor shall furnish all labor, tools, equipment, parts, materials and transportation to facilitate maintenance and cleaning of the interior and exterior surfaces of the entire kitchen hood and exhaust systems in the facilities listed below located at Whiteman AFB, MO. The maintenance and cleaning shall include the entire hood and exhaust system from the opening in the hood to the final air outlet including hood, fan housing, motors, all duct-work, rain caps, and bird and insect screens. After maintenance and cleaning, the hood and exhaust systems shall meet the standards specified in paragraph 10. If other circuits need to be turned off the contractors shall coordinate with the Facility Manager/Building Representative prior to work being started. If a person is to work inside the duct, another person must be stationed nearby as an additional safety precaution. When a person is working inside a duct or fan near an open access panel, proper ventilation shall be provided. All drop lights shall be protected against breakage. Scaffolds and ladders may be used to accomplish the cleaning. All scaffolds and ladders shall be equipped with rubber feet (or similar) to eliminate damage to floor surfaces. Flammable solvents or other flammable cleaning aids shall not be used. Precautions shall be taken to prevent damages to the automatic fire control system over the hoods. OSHA: While on Whiteman Air Force Base, all contractor personnel shall comply with applicable safety requirements of the Occupational Safety and Health Act (OSHA). OPERATION REQUIREMENTS: 5.1 Hood and Duct Cleaning/Maintaining Checklist 1) Insure the following items are free of grease and residue (bare metal): (a) Roof vent caps (b) Louvers (c) Fan motor housing (exterior) (d) Surface of fan blades (e) All ducts lengths (f) Hood and canopy opening (g) Roof catch pans 2) Insure the following areas are free of grease and residue: (a) Interior work areas (b) Roof and gutters 3) Insure there is no grease seepage through ducts to the exterior. Clean all seepage if found. 5.2 Cleaning Grease Exhaust Systems 1) All accumulated grease, dirt and debris shall be cleaned from all interior portions of the grease exhaust system (vents, ducts, hoods, dampers, etc.) beginning at opening of vent above filter banks, including fan, fan housing, louvers and roof catch pans. Gaylord Systems shall be maintained according to the Factory Operations & Maintenance Manual provided by 509 CES/CEOES. 2) The chemical cleaning shall be one (1) part calcium and two (2) parts calcium carbonate or equal in accordance with NFPA Fire Protection Handbook. The above mentioned hood and duct cleaning checklist shall be used to insure all phases of the cleaning contract will meet US Air Force requirements. 3) Components of the fire suppression system shall not be rendered inoperable during the cleaning process. At the start of the cleaning process, electrical switches that could be accidentally activated shall be locked out. Replace cover plates and adjust dampers and diffusers in correct position for proper airflow. All bolts, screws and nuts removed by the contractor from exhaust fans on roofs shall be replaced to insure that the fans are secure. 5.3 Repair Estimate 1) If, during maintenance, operating condition of a hood or exhaust system is found to be unsatisfactory, and repairs are required, a written statement of corrective actions and cost estimate shall be submitted to 509 CES/CEOES. The statement shall include an estimate of all labor, parts, materials and equipment needed. 2) No repair action nor parts ordering shall be initiated by the contractor without separate approval from the Contracting Office. If the contractor performs any repairs prior to written approval by 509 Contracting Officer, the contractor assumes all costs and responsibilities at no extra charge to the government. 5.4 General Instructions 1) All volume dampers in duct systems shall be reset to their original positions upon completion of the work. The contractor will clearly mark the setting of the duct before cleaning is started. 2) The contractor shall leave all roofs and gutters and other areas of his work free from grease or other substances used in or resulting from operations under this contract. 3) Drop cloths shall be used under the hoods and duct openings while work is being performed. 4) All grease, cleaning liquids and other debris shall be disposed of by dumping in off-base sanitary landfill. 5) Samples and literature for all chemicals to be used by the contractor shall be submitted to the government for approval prior to being used in connection with this contract. CONTRACTOR PERSONNEL REQUIREMENTS: 1) All personnel employed by the contractor shall be skilled employees qualified in this type of work. The contractor shall be responsible for all damages to Government property resulting from negligence on the part of the contractor or his employees. Any government property contaminated and not properly cleaned or items damaged shall result in replacement by the contractor or reasonable payment withheld from the contractor. 2) The attire of all contractor personnel shall be as neat and clean as practical during the performance of their duties. MAINTENANCE, CLEANING AND INSPECTION SCHEDULE: 1) Items scheduled for cleaning shall be serviced as scheduled in this Statement of Work. 2) The contractor shall notify the Base Fire Department Alarm Room at (660) 687-4507/08, a minimum of 48 hours prior to starting any work. Approximately one (1) week notice shall be given to the Civil Engineer Maintenance Engineering office (660) 687-2557/2627 for the purpose of scheduling contract work with the using agencies. This procedure shall apply to each service trip. 3) Normally, the service to render under this contract shall be performed Monday through Friday, excluding federal holidays, between the hours of 0700 and 2400. Work will be scheduled and coordinated with the Civil Engineer Fire Department Technical Services office on days and times mutually acceptable, not to interfere with normal operating hours/work of the facility. 4) Inspection on each grease exhaust system will be accomplished and discrepancies, if any, shall be corrected prior to placing exhaust system into service. When the contractor commences work on any grease system, the work must be continued until that exhaust system has been completed and accepted by the Contracting Officer or a designated government representative. SERVICE TICKETS: 1) Upon completion of service required by this contract for each exhaust system, the contractor shall prepare a service ticket which will reflect the following information: a) Contract Number b) Building Number Serviced c) Contract Line Item Number d) Date e) Description 2) Signature of Inspector/QAE denoting acceptance will be obtained and a duplicate copy of the service ticket shall be furnished to the Inspector/QAE at this time. 3) A copy of each service ticket shall be submitted to: 509 CES/CEOES 660 10th Street, Suite 139 Whiteman AFB, MO 65305 INVOICES: Invoices shall be submitted through Wide Area Work Flow QUALITY STANDARD: After maintenance and cleaning, the entire hood and exhaust system from the opening in the hood to the final air outlet shall be clean and free of all grease, dirt, lint, sludge and other contamination. The work area and all kitchen surfaces and equipment shall be free of all cleaning materials and all residues, dirt and grease resulting from the cleaning operation. The contractor shall insure the hoods/ducts are operating as specified by the manufacturer after each maintenance and cleaning service. After work is done, inspection shall be conducted by the QAE, Fire Department Representative and Facility Manager/Building Representative and all unsatisfactory results shall be corrected at the same time at no cost. SECURITY: Bldg 34, In-Flight Kitchen, is located in the secure "B Area" and will require submission of a letter from the contracting office for processing an Entry Authorization Letter (EAL) at least two (2) weeks prior to the performance date. The following information on a company letterhead shall be submitted: Contract number, date and time of service, facility name and number, full names of contractor personnel, dates of birth, social security numbers, driver's license numbers and state of issue, complete vehicle description to include make, model, year, color, license plate number, and state of registration. The contractor shall bring the vehicle registration, vehicle insurance card and two (2) forms of ID for each individual to gain access at Whiteman AFB. The contractor shall also provide this information to CE Maintenance Engineering. This requirement is mandatory for service in the designated facility. Information requested includes successful execution of contracts, and in problem areas, the steps taken to resolve or correct. (a) Content. 1) Offerors shall submit a consecutive list of all past and present performance for the last three years. Offerors shall submit the list no later than the date and time indicated the combined synopsis/solicitation with their quote, to A1C Dan Blackmore (contact information below.)
