Statement of Work (SOW) Credit History and Transaction Reporting 1.0IntroductionThe mission of the National Indian Gaming Commission (NIGC) is to ensure the integrity of gaming activities on Indian lands and to make certain that tribes are the primary beneficiaries of these gaming activities. The National Indian Gaming Commission requires the services of a credit bureau that maintains current and accurate data on the credit history of casino employees.as well as those patrons that have been extended credit at the various Indian casinos. This information is a valuable resource for NIGC in the conduct of mandated background and suitability investigations on individuals working in the gaming industry. 2.0 Objectives The purpose of this service is to ensure that NIGC has the proper access to gaming credit reports on individuals who have applied for and/or have been extended credit at gaming casinos around the world. Access to this specific information provides the NIGC a necessary resource in its responsibility to conduct background investigations on individuals who have a financial interest in, or management responsibility for, a management contract. 3.0Scope of Work The vendor will provide accurate and up-to-date credit reports to NIGC in a timely manner upon request per industry standard. The number of credit report requests is not anticipated to exceed 50 in a 12 month period. 4.0 Contract Type/Inspection 4.1 The resulting purchase order shall be a firm-fixed price for each credit report, for up to 50 credit reports per year for the base and each resulting option year 4.2 Inspection/Acceptance is defined in accordance with FAR 52.212-4(a). 5.0 Period of Performance The period of performance shall be for 1 Base Year and 4-1 Year Option Years, from the date of award. 6.0 Privacy Act Work on this project requires that NIGC personnel have access to Private/Confidential Information. All NIGC personnel shall adhere to the Privacy Act, Title 5 of the U.S. Code, Section 552a and applicable agency rules and regulations. 7.0Coordination and Monitoring The Director of Administration and the Director of Contracts of the NIGC shall monitor and coordinate performance under this contract. 8.0Contract Administration NIGC staff will not be authorized to order or accept work beyond the scope of the contract or to obligate in any way additional work under or outside the contract. Proper authority for any contractual changes will be granted through the National Business Center's Contracting Officer. Questions regarding the technical nature of the contract may be directed to NIGC's Director of Administration or the Director of Contracts. (2) To constitute a proper invoice, the invoice must include the following minimum information and/or attached documentation: "Name of business concern/letterhead;"Invoice date and separate/distinct number;"Contract and Task Order number;"Description of services and dates upon which services were rendered;"Payment terms;"Name, title, phone number and complete mailing address of responsible official to whom payment is to be sent or to contact in case of a defective invoice;"Total charges for the billing period. 52.217-9 -- Option to Extend the Term of the Contract (Mar 2000) (a) The Government may extend the term of this contract by written notice to the Contractor within 10 days [insert the period of time within which the Contracting Officer may exercise the option]; provided, that the Government gives the Contractor a preliminary written notice of its intent to extend at least 10 days before the contract expires. The preliminary notice does not commit the Government to an extension.(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 Years. (End of Clause) PROVISIONS 52.217-6 -- Option for Increased Quantity.As prescribed in 17.208(d), insert a clause substantially the same as the following:Option for Increased Quantity (Mar 1989)The Government may increase the quantity of supplies called for in the Schedule at the unit price specified. The Contracting Officer may exercise the option by written notice to the Contractor within 10 days. Delivery of the added items shall continue at the same rate as the like items called for under the contract, unless the parties otherwise agree. FAR 52.252-2, CLAUSES INCORPORATED BY REFERENCE (FEB 1998) This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text.
