Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; The provisions and clauses incorporated herein are those in effect through Federal Acquisition Circular 2005-26, effective June 12, 2008. The Department of Veterans Affairs Medical Center/Employee Education Resource Center, Cleveland, Ohio proposes to enter into a non-competitive commercial item contract with The Institute for Healthcare Improvement 20 University Road, Cambridge, MA. The scope of the project is for The Department of Veterans Affairs to participate in an international improvement process where teams will set local aims for improvement, connect with ideas from other healthcare systems from around the world, and test the changes within the Veterans Affairs patient population that includes congestive heart failure and chronic lung disease and patients with diabetes who live in rural areas. The Triple Aim Teams will assist VHA meet three critical objectives: Improve the health of the target populations for each respective team; Enhance the patient experience of care (including quality, access, and reliability) and reduce, or at least control the per capita cost of care. During the prototyping period, faculty shall conduct site visits at facilities that may volunteer. There shall be two all team meetings on in the Fall, 2008 and another one in the Spring, 2009. In these meetings, the teams will present the designs they are testing to the faculty and to each other and gather suggestions from the faculty and other teams on how to strengthen their testing of new designs and the full execution of these designs.The period of performance will be September 1, 2008 through June 1, 2009. This solicitation is unrestricted and The North American Industry Classification System (NAICS) is 611430 and the small business size standard is $5.0 million. A market survey was conducted and there is a reasonable basis to conclude that the services required are only available from this source. The proposed contract action is for services for which the Government intends to solicit and negotiate with only one source under the authority of FAR 6.302. Interested persons may identify their interest and capability to respond to the requirement or submit proposals. This notice of intent is not a request for competitive proposals. A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. The following provisions and clauses shall apply to this solicitation: 52.252-2 Clauses Incorporated by Reference. This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make their full text available. Offerors must include a completed copy of the FAR 52.212-3 Offeror Representations and Certification along with his/her proposal; FAR 52.212-4 Contract Terms and Conditions Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statutes on Executive Orders Commercial Items and in accordance with FAR 12.603(c) (2)(xii) the following provisions under the clause apply (b)(11)-(b)(15), (b)(19)(i), (b)(21), (b)(25) (b)(26) applies if Government Purchase Card is used, (c)(1), (c)(2), and VAAR 852.203-70 Commercial Advertising; 852.237-70 Contractor Responsibilities; and 852.270-1 Representatives of Contracting Officers.
