- The Centers for Medicare and Medicaid Services (CMS) intends to award on a sole source basis, under the Simplified Acquisition Procedure (dollar threshold of $100,000), for expert services from the American Academy of Family Physician, PO Box 11210, Shawnee Mission, KS 66207-1210. Under the authority of FAR 6.301-1 10 U.S.C. 2304 only one responsible source and no other supplies or services will satisfy agency requirement. The purpose of this request is to procure expert services from the American Academy of Family Physicians (AAFP) to provide education and outreach expertise as it relates to the implementation of physician quality measures used in CMS’ pay for reporting and Value Based Purchasing (VBP) programs. Membership in AAFP is comprised of primary care and subspecialty physicians who provide comprehensive care to millions of Medicare patients. CMS will identify opportunities to align its education and outreach priorities with educational activities of this medical specialty society to facilitate an efficient, national strategy for educating primary care physicians and their staff in implementing physician performance measures used in the Physician Quality Reporting Initiative (PQRI). The AAFP has been identified as a recognized national organization which is actively taking a leadership role within the AQA and NQF and which can provide a comprehensive approach to education and outreach efforts to engage primary care physicians in quality measurement and participation in CMS PQRI. AAFP –Representing more than 94,000 family physicians and medical students nationwide, the Academy maintains a variety of quality improvement tools and resources to assist physicians and medical students in implementing quality improvement projects and practice re-design. These tools include: The Practice Enhancement Forum, a quality improvement initiative developed to improve patient care by providing family physicians and their office staffs with education and hands-on assistance with QI, office redesign and implementation of the Chronic Care Model (CCM), offered at the AAFP constituent chapter level in cooperation with local QI experts; METRIC (Measuring, Evaluating and Translating Research Into Care), an online practice improvement program that allows family physicians to earn CME credit in their office for improving patient care and to fulfill the maintenance of board certification requirements, TransforMED, a practice redesign support service to assist family physicians to implement a medical home concept. CMS has a need to improve physician understanding and participation in PQRI. The majority of PQRI measures address critical aspects of care provided to millions of Medicare beneficiaries by primary care physicians. CMS is committed to initiating a national strategy for primary care physician in PQRI. In 2007 only 16% of eligible professionals participated in PQRI. CMS goal for 2008 is to increase participation by 50%. In order to reach this goal, it is critical for CMS to reach out to physician specialty groups and charge them with educating their membership about the benefits of participating in PQRI. Leveraging CMS’ national outreach efforts to take advantage of established specialty society activities at the chapter-level will promote faster, more efficient education and understanding about effective implementation of PQRI performance measures. Establishing a collaborative effort with medical specialty societies will support and advance public accountability, quality improvement, increased patient safety and value-based purchasing and will enhance the care of Medicare beneficiaries. AAFP has broad participation from all parts of the healthcare system, through national, regional, state, and local chapters representing, physicians, medical students and their practice managers. All responsible sources may submit capability statement, which shall be considered by the agency in accordance with the requirements in the synopsis.
