Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. It is the intent of the Syracuse VA Medical Center 800 Irving Ave. Syracuse, NY 13210, to negotiate a sole source procurement with Ber National Controls, Inc., 105 Arterial Road, Syracuse, NY 13206. This Replace Gates and Readers Requirement is to provide all labor, materials and equipment, inclusive of demolition and removal of existing equipment and materials and replacement/repair of the same areas, locating existing utilities and as described below: 1.Parking Lot # 6 : Provide new reader only, complete gate unit, traffic loops, logic & digital controls and reuse of existing power & com wire service to lot 6 site. (Quote, $___________) 2.Parking Lot # 5: Provide new Gooseneck/reader, complete gate unit, traffic loops, logic & digital controls & new com wire to lot 5 site. Reuse existing power wire service. (Quote, $___________) 3.Parking Lot #4 : Demo existing gate base. Provide new Gooseneck/reader, 4'd x 24"w concrete base, complete gate unit, traffic loops, logic & digital controls and AC power & com wire and underground conduit homeruns from Bldg. 16 to the lot #4 site. (Quote $___________) 4.Parking Lot #1: Provide new 4'd x 24"w concrete base, complete gate unit, traffic loops, logic & digital controls and AC power & com wire, remote push button vending of gate and COMPLETE conduit homeruns for power and com wire. (Quote $___________) Lot #1 is a NEW site. 5.Lower Parking Garage: Provide (2) new Gooseneck/readers, (2) complete gate units, traffic loops, logic & digital controls and new com wire to each gate site, with remote push button vending of each gate. Reuse existing power wire service. (Quote, $____________) 6.Upper Parking Garage: Provide new readers only, corroded or damaged gate repair materials, traffic loops and remote push button vending of each gate. (Quote, $___________) All new proximity readers in this project will be connected back to the existing hospital based system software. The new exterior equipment provided must be corrosion resistant and compatible with the existing software. Ber-National Controls installed the existing software and must configure the database to program and control each parking lot. Each reader must be interfaced back to the existing micro 5 control panels. The contractor will coordinate with VA Security on the programmable times of access and restriction for each lot. All new or repaired systems shall be fully tested and proven to function as intended. 3- O&M manuals with a minimum of 4 hours training will be provided. All communications between all elements of this system shall be verified by a manufacturer's representative. New Gates shall be Federal APD Model G-90 Series. Card Reader Gooseneck stands will also be Federal APD. New Proximity Card Readers shall be Casi-Rusco Model 940. All electrical work shall be performed per NEC standards. Existing reader and gate conduit pathways inside of buildings may be re-used. Each reader and gate must have it's own conduit path. No other sources can fulfill requirements to Replace Gates and Readers. This procurement is being conducted under the authority of Far 6.302-1. Only one responsible source and no other equipment or training will satisfy agency requirements; therefore, full and open competition is determined inappropriate for this acquisition. This notice is for information only and may represent the only official notice of such solicitation/synopsis, however, the Government will consider all proposals received on or before due date identified below. A determination by the Government not to compete this proposed procurement based on the responses to this notice is solely within the discretion of the Government. The provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this solicitation. The provision at 52.212-3, Offeror Representations and Certification - Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its proposal. A copy of the provision may be attained from http://www.arnet.gov/far/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. The offeror shall also provide its Commercial and Government entity code, DUNS number, and Tax Identification number. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of the information added to CCR. To register, go to http://www.ccr.gov/. The Federal Contractor Program requires that any contractor receiving a contract in the amount of $25,000 or more, or any subcontractor receiving a contract in the amount of $25,000 or more from such a covered contractor must file a VETS-100 Report on an annual basis.
