Description: THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR A PERFORMANCE BASED FIRM-FIXED PRICE CONTRACT WITH COST REIMBURSEMENT LINE ITEMS FOR TRAVEL, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE ONLY SOLICITATION. PROPOSALS ARE BEING REQUESTED AND A WRITTEN SOLICITATION WILL NOT BE ISSUED. Solicitation No. 2008-n-10433 is being issued as a Request for Proposals (RFP). RFP No. 2008-N-10433. The associated NAICS code for this acquisition is 541690. This will be a Full and Open Competion The purpose of this contract is to secure a phased-in approach for NHBS and MMP data management services. Specific data management objectives include the following: 1. Receiving data from no more than 25 NHBS and 26 MMP sites (CDC designated sites) 2. Processing data for quality assurance 3. Creating and transferring cumulative and final data sets to CDC and to project areas 4. Providing ad-hoc technical assistance to NHBS and MMP project areas 5. Providing formal training sessions for NHBS and MMP project areas 6. Communication and reporting to CDC The anticipated Period of Performance is 36 months. Solicitation No. 2008-N-10433, and all attachments will be available for download on the Federal Business Opportunities website at www.fedbizopps.gov. Additionally, all amendments and responses to questions will be posted on the Federal Business Opportunities website. The Government reserves the right to cancel this solicitation, either before or after the closing date. All questions must be submitted in writing to Sharon Cunningham at cux0@cdc.gov.
