Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a notice of intent to establish multiple Blanket Purchase Agreements (BPAs) for the procurement of Cadavers through the Willed Body Program. The purchase of cadavers falls under the jurisdiction of the Texas State Anatomical Board as stipulated in the Texas Health and Safety Code, Chapter 691; Anatomical Board of the State of Texas, and Chapter 692: Texas Anatomical Gift Act. Cadavers can only be purchased within the State of Texas from medical or dental schools. Solicitation W81K04-08-Q-4321 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16. This acquisition is being conducted as a commercial item acquisition in accordance with FAR Part 12. This requirement is unrestricted and all responsible sources may submit a quotation, which will be considered under NAICS code 621991. The requirements of this solicitation is to provide embalmed and unembalmed cadavers for the U.S. Army/Baylor Doctoral Program in Physical Therapy at the U.S. Army Medical Department Center and School, Fort Sam Houston, Texas. The number of cadavers and delivery dates will vary. The delivery of unembalmed cadavers is at least two days before class dates. At the end of the course, cadavers shall be returned to the contractor of origin for cremation. There are no age limits for a cadaver. Shipping is only by authorized methods; a mortuary service. The Willed Body Program is obligated to follow the Body Bequeathal Agreement made with the family members to either return the cremated remains to the family members or for the contractor to provide a burial ceremony on behalf of the family, as agreed upon, at the time when the request was made. Embalmed Cadavers 2. Unembalmed Cadavers, 3. Fresh Heads; 4. Delivery to the U.S. Army Medical Department Center and School, Fort Sam Houston, Texas, 5. Cremation 6. Administrative Fee, 7. Hepatitis B & C Testing, 8. HIV Testing, 9. Assessment Fee. All quotes must be submitted electronically to lisa.riche@us.army.mil and faye.shepherd@us.army.mil or via fax to 210-221-5402 and must be received no later than Tuesday, 29 July 2008, 4:00pm CST. The RFQ number W81K04-08-Q-4321 must be listed on the submission. All vendors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed at Internet address: www.ccr.gov/. Confirmation of CCR registration will be validated prior to award. All payments issed for supplies received against this requirement will be made through the Government Purchase Card (GPC) or Wide Area Workflow (WAWF).
