- This is a combined synopsis/solicitation for comercial item prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number for this Request for Quote (RFQ) is Q4130080604. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisiton Circular (FAC) 05-26. Federal Acquisition Regulation (FAR) clauses and provisions are available electronically at htt://www.arnet.gov/far/. This acquisition is 100% set aside for small business. The associated NAICS code is 442210 and the small business size standard is no more than 500 employees. Contract Line Item Numbers (CLIN) to be issued at time of award: CLIN#0001ALPO Lemon House provide 281 sq yds of carpet x $______ per sq yd = $________ CLIN#0002JOFL Visitor Center provide 354 sq yds of carpet x $______ per sq yd = $________ The following options will be reviewed at time of award. The Government reserves the right to exercise or not to exercise options requested as part of the award. Vendors are required to provide pricing for all options in order for their quote to be considered for an award. Option 1: ALPO Lemon House remove and recycle 281 sq yds of existing carpet and pad being replaced. Includes moving existing office furniture and fixtures to remove floor covering 1 job = $________ Option 2: ALPO Lemon House install 281 sq yds of new carpet squares using non-toxic/bio-based carpet adhesive and place office furniture and fixtures 1 job = $_______ Option 3: JOFL Visitor Center remove and recycle 354 sq yds of existing carpet and pad being replaced. Includes moving existing office furniture and fixtures to remove floor covering 1 job = $________ Option 4: JOFL Visitor Center install 354 sq yds of new carpet squares using non-toxic/bio-based carpet adhesive and place office furniture and fixtures 1 job = $_______ In addition to the information already provided, the following applies to all aspects of this requirement as applicable: carpet squares or carpet tiles are to be greenish-gray or gray-green in color, minimum 40% recycled content, with high traffic design. Carpet sample must be provided with your proposal in order for your quote to be considered and the National Park Service to make a choice prior to award. All associated work will take place after 5:00pm to minimize operational disruption during week days or weekends. No work will be done on a Federal Holiday. Provide manufacturer's information showing material content, proper care, and warranty information for the carpet (product) with submission of your quote. The provisions at 52.212-1 Instructions to Offerors-Commercial, applies to this acquisiton. Evaluation procedure to be used will be lowest, responsible offer received that meets the requirements defined. Offerors are advised to include a completed copy of the provisions at 52.212-3, Offeror Representations and Certifications-Comercial Items with their offer. This may be completed electronically at http://orca.bpn.gov. FAR Clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition.
