- There is NO attached List with this Pre-solicitation. This is a pre-solicitation notice for Buckley AFB, CO to obtain qualified and experienced Registered Nurses or Clinical Social Workers, with an unrestricted and current license from within U.S. jurisdiction, to set-up and implement a local, on-site Case Management and Discharge Planning, Program at the U.S. Medical Treatment Facility(s) (MTF). The MTF Case Management and Discharge Planning (CM DCP) Program shall provide holistic patient-centered, individual and family level case management and discharge/disposition planning for beneficiaries in support of peacetime and military readiness and contingency requirements. The required services are commercial in nature, therefore, FAR Part 12, Acquisition of Commercial Items shall apply to this acquisition. The services must also, meet the qualifications in accordance with (IAW) all applicable Department of Defense (DoD), Air Force, Air Force Space Command (AFSPC), 460th Space Wing/Headquarters Chapel (460th SW/HC), regulations and policies. The contract period of performance begins 01 September 2008 and will run through 30 August 2009 with no option periods. Partial payments are not authorized. The requirement will be a 100% Service Disabled Veteran Owned Set-Aside under North American Industrial Classification System Code 621111 with a size standard of $9.0 million. The entire solicitation will be made available only on the Federal Business Opportunities website http://www.fbo.gov on approximately 01 Aug 2008. No paper copies will be issued.
