Description

Travis AFB intends to award a sole source purchase order for a Roche Diagnostics Corporation for David Grant Medical Center. All responsible sources may submit a quotation, which shall be considered by the agency. The solicitation number is F3Z4538120AQ01. Travis AFB intends to award this sole source requirement to the Roche Diagnostics Corporation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-26; Defense DCN 20080721. Air Force AFAC 2008-0605. This acquisition will be sole source; the North American Industry Classification System (NAICS) code is 811219. The business size standard is $6.5M. The Federal Supply Class (FSC) is J065. The Standard Industrial Classification (SIC) is 3841. This request for quotation consists of the following items on an all or none basis: Item 0001: CONTRACT MAINTENANCE SUPPORT AGREEMENT FOR DIAGNOSTIC LABORATORY EQUIPMENT TO INCLUDE: Magna Lyser Serial No. 40467546 LightCycler 2.0 Serial No. 1407644 LightCycler 480 Serial No. 20436 Magna Pure Serial No. LCPG0571 Magna Pure Comp Serial No. MPCB0386 Period of Performance: Date of award to 12 months plus 4 one year options. The following provisions and/or clauses apply to this acquisition. FAR 52.212-3, Offeror Representations and Certifications-Commercial Items. An offeror shall complete only paragraph (j) of this provision if the offeror has completed the annual representations and certificates electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (b) through (i) of this provision.
