Description
This is a combined synopsis/solicitation for commercial buys, prepared in accordance with format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a separate written solicitation will not be issued. This solicitation: No. RFQ-NCI-80108-NG-01 includes all applicable provisions and clauses in effect through FAR FAC 2005-26 (June 2008) simplified procedures for commercial items. This acquisition will be conducted under the procedures for acquiring commercial items authorized in FAR Part 12 and under the authority to use simplified procedures for commercial requirements as provided in FAR 13.501. The North American Industry Classification System code is 541990 and the business size standard is $7.0M. The National Cancer Institute (NCI), Division of Cancer Epidemiology and Genetics (DCEG) plans to procure services to estimate the burden of prostate disease in Accra, Ghana for the Ghana Prostate Cancer Survey. Statement of Work - Ghana Study – Case Expansion A. Background Information General description: The NCI Division of Cancer Epidemiology and Genetics has conducted a population-based screening of prostate cancer in Accra, Ghana to investigate the burden of prostate diseases in Africans and the role of genetic susceptibility of prostate cancer in Africans. How the requirement evolved: In order to investigate the role of genetic susceptibility, the NCI needs to increase sample size of cases from 72 to about 400. Therefore, we are expanding the case recruitment of the Ghana cases to include cases diagnosed between 2005 and 2009. B. Purpose and Objectives Project objectives: The primary objective of the Ghana Prostate Cancer Survey is to estimate the burden of prostate diseases in Accra, Ghana. Specific objectives of this procurement: Objective of this procurement is to provide support for case expansion in the Ghana study by recruiting additional 300-500 prostate cancer cases into the Ghana study in order to evaluate the role of genetic susceptibility of prostate cancer in Africans. The contractor shall recruit prostate cancer patients diagnosed in Accra between 2005 and 2009. Contractor must have strong support from the urologic community in Ghana with full access to medical, surgical and pathology reports at the Korle-bu Hospital. Contractor shall have access to the clinical and interview data from the original study which are owned and stored at the University of Ghana. C. Contract requirements The contractor shall perform the following tasks: •Maintain a study office to coordinate case identification and recruitment •Identify eligible cases from 2005 to July 2009 •Maintain a list of study subjects •Contact and recruit eligible cases •Collect 20 ml of overnight fasting blood from eligible cases •Separate blood into various components, including buffy coat, plasma, serum, and blood clot based on the protocol provided by NCI investigator •Store these blood components at -70 C until shipping them to the NCI repository at Frederick. •Conduct interviews using the forms provided by NCI •Retrieve pathology reports and conduct medical record abstract using the form provided by NCI •Prepare and cut 20-30 pathology slides, depending on the availability, from each cancer case. Record block and specimen ID for each slide. •Abstract medical record and record treatment information for each case •Coordinate with NCI to ship specimens, including blood components and slides, to the NCI repository every 3 months •Ship biological samples on dry ice by Federal Express to the NCI repository •Report to NCI about case recruitment progress every month through either email or conference calls •Maintain a case ascertainment rate of greater than 85% D. Period of Performance: Period of Performance shall be from 01 SEP 2008 or date of award whichever is later for 12 months, with one 12 month option. E. Government Responsibility The government will be responsible for providing appropriate protocol, supplies (vacutainers, tubes, and bar-coded ID labels), and forms for the study to record data and for the transmittal of data. F. Reporting Requirements and Deliverables Deliverables include biological specimens, pathology slides, forms and electronic data needed to confirm the quality of data that have been received at NCI. Ship biological samples, including blood, urine, and histology slides, to the NCI repository (address to be provided at time of award). Biological samples, such as blood and urine, should be packed with dry ice and sent by air. Histology slides should be shipped by FedEx without dry ice. Deliverables shall be submitted quarterly. Send verified information by email to the study manager. Medical record abstract forms for cases need to be keyed and electronically transferred to NCI (see attached). For most data, Excel spread sheet can be used for data transfer. G. Inspection and Acceptance Criteria Upon receipt of the data, the NCI investigator will review the results to assure that they conform to the requirement set forth in this Statement of Work. This will be completed within two months of receipt of deliverables. BASIS FOR AWARD: Award will be made, subject to available funding and quoter responsibility, to that acceptable quote. All evaluation factors, other than price, when combined are significantly more important than price. However, price may become a critical factor in source selection in the event that two or more quoters are determined to be essentially equal following the evaluation of all factors other than price. In any event, the Government reserves the right to make an award to that quoter whose response provides the best overall value to the Government. The Evaluation Process: The Government will award a contract resulting from this quotation to the responsible contractor whose quote conforming to the quotation will be the most advantageous to the Government, price and other factors considered. The following evaluation factors shall be used to evaluate the prospective contractors and are listed in order of importance. 1. Technical Capability (50 points) 2. Personnel (30 points) 3. Past Performance (20 points) – this should be related to past performance on recruiting prostate cancer cases in Ghana 4. Price Past Performance Information: Quoters shall submit the following information as part of their response. •A list of the last three (3) contracts completed during the past five years and all contracts currently in process that is similar in nature to the solicitation. Contracts listed may include those entered into by the Federal Government, agencies of state and local governments and commercial concerns. Quoters/offerors that are newly formed entities without prior contracts should list contracts and subcontracts as required above for all key personnel. Include the following information for each contract or subcontract: 1. Name of Contracting Organization 2. Contract Number (for subcontracts, provide the prime contract number and the subcontract number) 3. Contract Type 4. Total Contract Value 5. Description of Requirement 6. Contracting Officer's Name and Telephone Number 7. Program Manager's Name and Telephone Number 8. Standard Industrial Code The quoter/offeror may provide information on problems encountered on the identified contracts and the quoter’s/offeror's corrective actions. Each quoter/offeror will be evaluated on its performance under existing and prior contracts for similar products or services. The Government is not required to contact all references provided by the quoter/offeror. Also, references other than those identified by the quoter/offeror may be contacted by the Government to obtain additional information that will be used in the evaluation of the quoter/offeror's Past Performance. Evaluation Criteria: 1Technical Capability (50 Points): The technical response should clearly and fully demonstrate the quoter’s full understanding of the capabilities, knowledge and experience, and personnel requirements, as stated in the statement of work. Failure to respond explicitly to each of the categories may result in the quoter’s offer/quotation being deemed technically unacceptable. The contractor shall provide evidence of: 1) collaboration with University of Ghana and Korle-Bu Hospital; and 2) capability to obtain pathology slides and clinical data to include medical records, pathology reports, and surgical reports, at Korle-Bu Hospital. 2Personnel (30 Points): Proposed Personnel (key personnel only) technical and professional qualification relevant to the tasks required for this contract. The contractor shall provide: Resumes with letters of commitment from personnel who will perform the tasks described in the SOW. 3 Past Performance (20 Points) The Government will evaluate the offeror's past performance based on information obtained from references provided by the offeror, including experience in recruiting, enrolling, and collecting biological smaples form prostate cancer patients in Accra, other relevant past performance information obtained from other sources known to the Government, and any information supplied by the offeror concerning problems encountered on the identified contracts and corrective action taken. The Government will assess the relative risks associated with each offeror. Performance risks are those associated with an offeror's likelihood of success in performing the acquisition requirements as indicated by that offeror's record of past performance. The Government will consider the currency and relevance of the information, source of the information, context of the data, and general trends in the offeror's performance. The lack of a performance record may result in an unknown performance risk assessment, which will neither be used to the advantage nor disadvantage of the offeror. The past performance subfactors are listed below in order of relative importance. These subfactors will be used to evaluate the quality of past performance. Past Performance Subfactors: •Record of conforming to specifications and to standards of good workmanship •Adherence to contract schedules, including the administrative aspects of performance •Reputation for reasonable and cooperative behavior and commitment to customer satisfaction •Business-like concern for the interest of the customer 4Price (Price will be evaluated separately but not scored).
