- (1)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are requested and a written solicitation will not be issued. (ii)This solicitation/synopsis reference number is F3M3C18211A001 and is being issued as a Request for Quote (RFQ). (iii)This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16. (iv)This acquisition is 100% set-aside for small businesses. The associated NAICS code is 541620 with a $6.5 Million size standard. (v)This requirement consists of one Line Item: Perform annual asbestos survey of approximately 49 buildings. Description is as follows: In 2004, the United States Air Force contracted a company to conduct a baseline survey to locate and identify asbestos-containing materials (ACM) in existing Pope AFB facilities. The company assessed the quantity and condition of ACM in each facility surveyed and provided a report documenting the findings. This report serves as the baseline from which an annual update will be performed and documented. The contractor will provide the labor and materials required to perform the Annual update to the baseline asbestos survey. Following the approach outline in the baseline survey, the contractor will: Review the existing baseline survey report and any other facility records which can be supplied by base personnel; Inspectors must be accredited for this work by the State of North Carolina to perform asbestos surveying, copy of the accreditation must be provided at time of award; Visually inspect each facility to assess the current condition of documented ACM; Utilize protocols established by the Asbestos Hazard Emergency Response Act (AHERA) Provide a report, similar in format and structure to the baseline survey, documenting all findings. Baseline survey is available for review; Provide specific recommendations that address any needed ACM maintenance or removal, by building, location, and type; Inspectors to install an adhesive sticker containing the month, year, and inspectors name at each location where asbestos is found. The project deliverables are: A final report, documenting the effort, findings, data, and recommendations as described in 2(e) and 2(f) above. Four hard copies and a CD-ROM of the final report. Project Schedule: The survey will be completed within 60 days from receipt of contractual Notice of Award; The final report will be delivered not later than 90 days from receipt of contractual Notice of Award. (vi) Pricing shall be FOB Destination.
