Description
Travis AFB intends to award a sole source purchase order for a TREK Diagnostic Systems for David Grant Medical Center. All responsible sources may submit a quotation, which shall be considered by the agency. The solicitation number is F3Z4538198A0004. Travis AFB intends to award this sole source requirement to the TREK Diagnostic Systems. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-26; Defense DCN 20080721. Air Force AFAC 2008-0605. This acquisition will be sole source; the North American Industry Classification System (NAICS) code is 339112. The business size standard is 500 employees. The Federal Supply Class (FSC) is 6515. The Standard Industrial Classification (SIC) is 3841. This request for quotation consists of the following items on an all or none basis: PERFORMANCE WORK STATEMENT/EQUIPMENT LISTING VersaTREK 240 1.0 PROVIDE RENTAL EQUIPMENT AND REAGENTS FOR THE VERSATREK 240 ANALYZER. 1.1 THE AGREEMENT INCLUDES ALL CONSUMABLES (I.E. REAGENTS, TEST-KITS, CONTROLS AND CALIBRATORS) UTILIZED DURING THE OPERATION OF THE INSTRUMENTS AND FULLY WARRANTED FOR 60 MONTHS COVERING ALL PARTS, LABOR AND TRAVEL. TECHNICAL SUPPORT IS AVAILABLE 24/7 AND SERVICE MAINTENANCE WILL BE PROVIDED MON-FRI, 8:00AM - 5:00PM, EXCLUDING HOLIDAYS. 1.2. Replacement of dysfunctional analyzers. 1.2.1 REPLACEMENT OF DYSFUNCTIONAL ANALYZERS WILL OCCUR WITHIN ONE WEEK OF MANUFACTURER NOTIFICATION. IF BOTH ANALYZERS WERE DYSFUNCTIONAL, A REPLACEMENT ANALYZER WOULD BE PROVIDED WITHIN 24 HOURS. 2.0 Instruments. Currently, the laboratory utilizes the Versa TREK 240 analyzer. 3.0 Training. Provide on-site training for the technicians as needed. Period of Performance: Date of award to 12 months. The following provisions and/or clauses apply to this acquisition. FAR 52.212-3, Offeror Representations and Certifications-Commercial Items. An offeror shall complete only paragraph (j) of this provision if the offeror has completed the annual representations and certificates electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (b) through (i) of this provision. FAR 52.212-3 shall be completed as noted above and submitted with the offer. Offerors must comply with all instructions contained in FAR 52.212-1 Instructions to Offerors - Commercial. As prescribed in 32.705-1(a), insert the following clause: Availability of Funds (Apr 1984) Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer.
