- In accordance with FAR Subpart 13.106-1(b), the Centers for Disease Control and Prevention (CDC) intends to issue a sole-source, purchase order contract to National Council on Aging (NCOA) in Washington, D.C. NCOA is the only organization overseeing a national Free Falls Coalition (FFC), and the nature of the activities outlined in the SOW require access to and leadership over the FFC. No other products exist at this time. NCOA is uniquely placed and qualified to perform these tasks. NCOA's membership include senior centers, area agencies on aging, adult day service centers, faith-based service organizations, senior housing facilities, employment services, consumer groups, and leaders from academia, business, and labor. The prospective contractor has extensive experience, background and knowledge to accomplish this endeavor because NCOA has implemented the FFC, a unique collaborative initiative to address the growing problem of falls and fall-related injuries among adults age 65 and older. The FFC formed in 2004, includes more than 55 organizations and employs a collective approach to promoting a national falls prevention action plan. In accordance with the Government's market research no other qualified vendor exists at this time. NCOA is uniquely placed and qualified to perform this task. NCOA's understanding of the FFC and credibility renders them the only know contractor capable of accomplishing this tasks within the Performance Work Statement. The forthcoming contract requires the contractor to develop communication mechanisms, implement interventions, and evaluate program effectiveness specific to falls and fall-related injuries among adults age 65 and older. Essential elements of support include monitoring FFC's progress in meeting its objectives. Successful completion of this project requires: (1) National Council on Aging (NCOA) participation at its highest leadership level. (2) Access to NCOA members. (3)Access to the FFC. (4) Proven knowledge and experience in injury prevention specific to falls, in both policy and scientific areas. (4) Demonstrated ability and experience in developing and implementing falls prevention strategic plans cleared and implemented by NCOA and NCIPC. (5) Demonstrated ability and experience in developing and implementing national level falls prevention materials. Interested parties who feel that they possess the capabilities to provide the services described above should submit a capability statement demonstrating their abilities to provide the services. The information received in response to this notice will be considered solely for the purpose of determining whether or not to conduct a competitive procurement. All responses shall be submitted via email to MEAllen@cdc.gov with the reference number (00HCTC12-2008-59364 -- Fall Prevention Project) referenced in the subject line. Documentation should be received within fifteen (15) days after the date of publication of this synopsis to be considered by CDC. A determination by the CDC not to compete this proposed contract based upon responses to this notice is solely within discretion on of the Government.
