- The Veterans Integrated Services Network (VISN) 4 is seeking consultant services to assist VA Healthcare VISN 4 Community Living Centers change the culture from an institutional medical model to a resident-centered model. The goal of this initiative is to transform the current delivery of care from the traditional medical model to a model that focuses on the veterans holistic needs to improve quality of care and quality of life. This initiative will involve transforming the current Community Living Center culture to a resident-centered approach; empowering front line workers to help address issues related to the delivery of customized, high quality care; changing the environment to foster a homelike environment; and linking transformation of the Community Living Center culture to quality indicators. The North American Industry Classification System (NAICS) Code is 541611. The Request for Quotations (RFQ), number VA-244-08-RQ-0374, will be available on or about August 18, 2008. Questions concerning the Statement of Services must be addressed in writing and sent to the Contract Specialist by 10:00 a.m. August 22, 2008. Quotations are due by 10:00 a.m. August 29, 2008 to the VA Medical Center, 1400 Blackhorse Hill Road, Building 19 South, 542/90C, Coatesville, PA 19320-2096.
