- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. RFQ# W44DVQ82117709 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16. This requirement will be processed under Commercial Item Procedures IAW FAR Part 12. This procurement is unrestricted and all responsible sources may submit a quotation, which will be considered. The requirements in this solicitation are: Clin0001, 1ea #08073R CerviPath Patient Management System which includes: Swing Colposcope, 300mm Working Distance, 3 Step Magnification (3.75x, 7 5x, 15x) Halogen Lighting: Swing-o-matic, 5 wheel Rolling Base, Swing-o-matic Arm Assembly, 1yr warranty-parts & labor; Clin0002, 1ea #08706 Video Camera Adapter; & Clin0003 1ea #53321 Cervipath Computer Patient Management System which includes: 1.3 Mega Pixel Digital Video Camera (utilizes USB 2 Technology; Integrated Laptop Design-Smaller Footprint; Integrated Flat Screen Technology; Infared Red Printer Connection; 4800 DPI Photo Quality Printer; Pentium 4 Computers with Windows XP, & 40GB Hard Drive; Software: CerviPath Patient Mgmt System; Customizable Reports with diagrams that can be electronically annotated on screen; 3 Standard Reports; Multiple Page Reports; Database Fields are fully customizable; Manually register Image Magnification at time of Capture; Password Driven Permissions, create, review, modify, release, or lock the clinical exam reports; Fully encrypted and protected folder mail and image mail functions; System is networkable; Hippa Compliant, DICOM & HL7 Compatible; Multiple Filters available for image enhancement. #35834 Operator's Manual for Colposcope and #35348 Operator's Manual for Cervipath Management System. Mfr: CooperSurgical. Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. Clause 52.211-6 (and other clauses throughout this solicitation) can be viewed by accessing website http://farsite.hill.af.mil/. Delivery, within 30 days after receipt of order, shipping shall be FOB destination to Irwin Army Community Hospital, Ft Riley, KS 66442-5037. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to awarding a contract. Evaluation Process: All proposals will be evaluated in accordance with the following criteria: Technical Capability (compliance with the requirements of FAR Clause 52.211-6 if vendor offers and Equal Product) and Price, with Technical Capability being significantly more important than Price. Vendors shall include a completed copy of FAR provision 52.212-3 with their offer. The provision can be downloaded by accessing the website listed in this solicitation.
