Description

Travis AFB intends to award a sole source purchase order for a Autotransfusion Inc. for David Grant Medical Center because of the unique and urgent response services needed in the immediate area. All responsible sources may submit a quotation, which shall be considered by the agency. The solicitation number is F3Z4538177A003. Travis AFB intends to award this sole source requirement to Autotransfusion Inc.. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-26; Defense DCN 20080721. Air Force AFAC 2008-0605. This acquisition will be sole source; the North American Industry Classification System (NAICS) code is 621999. The business size standard is $9M. The Federal Supply Class (FSC) is Q508. This request for quotation consists of the following items on an all or none basis: Item 0001: PROVIDE NON-CARDIAC PERIOPERATIVE AUTOLOGOUS BLOOD SALVAGING & PROCESSING SERVICES TO DAVID GRANT MEDICAL CENTER PATIENTS ON AN ON-CALL BASIS 24/7 DAYS PER WEEK AS REQUIRED. Period of Performance: 1 Oct 08 - 30 Sep 09. The following provisions and/or clauses apply to this acquisition. FAR 52.212-3, Offeror Representations and Certifications-Commercial Items. An offeror shall complete only paragraph (j) of this provision if the offeror has completed the annual representations and certificates electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (b) through (i) of this provision. FAR 52.212-3 shall be completed as noted above and submitted with the offer. Offerors must comply with all instructions contained in FAR 52.212-1 Instructions to Offerors - Commercial. As prescribed in 32.705-1(a), insert the following clause: Availability of Funds (Apr 1984) Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer.
