- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Purchase Request Number: W51HQT81560001 is issued as a request for quoation. (RFQ). This acquisition is set-aside 100% for small business under NAICS code 532490 and the small business size standard is 6.5 Million. The requirements in this requistion are: CLIN 0001, Lease Operating Cost on the Automated Prescription System (3) Prostalease Productivity Stations (Outpatient Pharmacy ) 12 months; CLIN 0002, (1) Prostalease Productivity Station (Commissary) 12 months; CLIN 0003, Pharmacy 1000 (Software Only) 12 months; CLIN 0004, Pharmacy 1000 (Software Only) 12 months; Autolink 5% for 12 months; CLIN 0006, Accumed 8% for 12 months. This requirement is for a base year starting 01 October 2008 with 2 Option Years till September 30, 2011.
