This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation W81NTE8168N529 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. Great Plains Regional Contracting Office has a requirement for NON-PERSONAL SERVICE SUPPORT TEAM COORDINATOR (Family Ministry Volunteer Coordinator) (NTE 12 Months). For the Ministry and Pastoral Care, Ft. Sam Houston, TX. This requirement is small business set-aside and all responsible sources may submit a quotation, which will be considered. Refer to: PERFORMANCE BASED WORK STATEMENT STATEMENT OF WORK FAMILY MINISTRY SUPPORT TEAM COORDINATOR FOR ALL VOLUNTEERS 1. GENERAL. 1.1. The Department of Ministry and Pastoral Care, Brooke Army Medical Center (BAMC), Fort Sam Houston has a requirement for the non-personal services of a Family Ministry Support Team Coordinator for all Volunteer activities programmed through the Department of Ministry and Pastoral Care. 1.1.1. The contractor must be capable of managing and interacting with volunteers, families, staff, and the volunteer staff chaplain. Coordinator must demonstrate competence by interviewing with Chief, Department of Ministry and Pastoral Care or his designee, and the BAMC DMPC volunteer staff chaplain. 1.2. The contractor must be able to coordinate family ministry activities, integrate effectively with BAMC DMPC staff, volunteers, Catholic volunteer LEMs, and other BAMC professional staff to accomplish the mission of BAMC DMPC. 2. DEFINITIONS. 2.1. CONTRACTING OFFICER: Person with the authority to enter into, administer, terminate contracts and make related determinations and findings. 2.2. CONTRACTING OFFICER REPRESENTATIVE (COR): A government employee selected and designated in writing by the Contracting officer to act as his designated representative in administering a contract. 2.3. CONTRACTOR: A person or business firm offering services to the Government under this contract. 2.4. STAFF CHAPLAIN: Deputy Chief, DMPC. 2.5. VOLUNTEER STAFF CHAPLAIN: Chaplain on the DMPC staff who is designated as the chaplain coordinator for all volunteer activities. 3. PERFORMANCE EVALUATION MEETINGS. 3.1 The contractor shall meet with the Chief, Department of Ministry and Pastoral Care, BAMC, or his designee at least once per week. The contractor shall coordinate through the BAMC DMPC volunteer staff chaplain to make appointments for this meeting. 3.1.1. The purpose of these meetings will be to discuss the ministry needs assessment of the BAMC Chief, DMPC and needs for population ministered to through volunteers. This includes program evaluation, and program/activity scheduling. The contractor and/or the COR shall notify the Contracting officer of any discrepancies, disputes, or indications of poor performance. The Chief, DMPC, if not the COR shall notify the COR of such indications. 4. SPECIFIC REQUIREMENTS. 4.1. The contractor shall arrange for a volunteer representative to attend BAMC Volunteer Council Meetings. 4.2. The contractor shall facilitate communication between team leaders, staff, and the BAMC volunteer staff chaplain. 4.3. The contractor shall contact all volunteers for support to special DMPC events and participation in mandatory training to remain certified for volunteer work BAMC. 4.4. The contractor shall coordinate and implement special events designated by Chief, DMPC. 4.5. The contractor shall ensure the BAMC volunteer staff chaplain is informed of problems, concerns, or improvement initiatives identified by team leaders. 4.6. The contractor shall inform the DMPC volunteer staff chaplain of vacancies. 4.7. The contractor will assure maximum participation in training, clear dates for event planning, and prevent duplication and other conflicting schedules. Contractor will assure volunteers comply with patient confidentiality requirements. 4.9. The contractor will introduce new volunteers to the program and assure volunteers are assigned to positions that complement abilities of volunteer and needs of DMPC. 4.10. The contractor shall perform and document all services in accordance with U.S. Army and BAMC regulations and policies, to include safety and fire policies. 4.11. The contract shall refer requests involving military administrative actions to the BAMC DMPC Staff Chaplain. 4.12. The contractor shall attend in-service training when deemed necessary by the BAMC DMPC Staff Chaplain or as directed by the Chief, DMPC. 4.13. If the contractor becomes ill or is otherwise unable to fulfill the obligations of this contract, the BAMC DMPC Staff Chaplain shall be notified as soon as possible. It is essential that continuity of services be maintained. This contract has no provisions for paid absences and payment will be made only for services rendered. Travel will not be paid separately. 5. GOVERNMENT-FURNISHED PROPERTY AND UTILITIES. 5.1. Equipment. When available appropriate equipment will be provided for the support of the contractor only in the actual performance of services. Such equipment will not be considered for the sole use of the contractor. 5.1.1. Appropriate equipment includes: workspace, chair, computer, and small storage space for books, binders, etc. 5.2 Utilities. The Government will provide all utilities, such as electrical, water, to perform the services required by the contractor, at the site of delivery of the services on federal property. The government will not make any modification to utility outlets to accommodate contractor equipment. 5.3 The Government shall not furnish the contractor with Government living quarters. 6. CONTRACTOR FURNISHED PROPERTY AND TULITIES. 6.1. The contractor shall furnish his/her own transportation to and from the work site. 6.2. The contractor may use personal items of equipment as part of his/her performance of duties with approval of BAMC DMPC Staff Chaplain. The Government will not be liable in any way for personal equipment including loss, damage, theft, or maintenance. 6.3. The contractor shall not be required to furnish any administrative support supplies in order to accomplish the requirements of this contract except as specifically noted. 7. ADMINISTRATION. 7.1. Definition. Administration for the purpose of this contract is the completion of reports, Government Forms, and other input, that may be required for each contract produce or service. 7.2. Documentation of Work/Reports. The contractor shall maintain a log of the time and work performed and under this contract, and shall provide a copy of the log to the Staff Chaplain on a monthly basis. The contractor will also submit a log of all DMPC volunteers hours on a monthly basis to a staff chaplain and DMPC Secretary. 7.3. The contractor shall document services performed under this contract on an Invoice or Request for payment through (WAWF) Wide Area Work Flow. See Clause Electronic Submission of Payment Request on Page 5 and 17 of this Purchase Order. 7.4. Request for Supplies/Equipment. The contractor shall provide the Staff Chaplain with the proper requests for ordering supplies and equipment to support the family ministry program in accordance with the Fort Sam Houston Consolidated Chaplains' Fund SOP (for Non-appropriate Fund supplies) and the BAMC DMPC Chaplain Resource manager's Instructions (for Appropriated Fund supplies). 7.4.1. The Fort Sam Houston Consolidated Chaplains' Fund Manager (for Non-appropriated funds) and the BAMC DMPC Resource Manager (for Appropriated Funds) will provide applicable forms, SOPs, and instructions to the contractor. 7.5. Privacy Act. The contractor shall be familiar with and insure compliance with provisions of the Privacy Act, Freedom of information Act, and limitations on Government collection of personal and statistical data on non-affiliated personnel. 7.5.1. Applicable Government regulations concerning these acts will be made available to the contractor for review by the BAMC DMPC Staff Chaplain. Clause 52.212-4 Contractor Manpower Reporting; Clauses throughout this solicitation can be viewed by accessing website http://farsite.hill.af.mil/ FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to awarding a contract. Evaluation Process: All proposals will be evaluated in accordance with the following criteria: Technical Capability (compliance with the requirements of Certification specializes in providing Clinical Pastoral Education; management of pastoral care departments; management of chaplains/Lay persons to include all aspects of religious services and certification programs) and Price.
