- The purpose of this amendment is to provide answers to questions regarding the solicitation. Q1: What are the specific instructions for submitting a cost/price proposal?A1: Please provide a cost proposal in the form of a loaded/burdened hourly rate. All travel required will be reimbursed at the current rate allowable in the Federal Travel Regulations on the date of travel. Q2: What are the formatting guidelines for the quote?A2: A resume providing information with regard to the qualifications in the statement of work and a summary of the understanding of the requirements is sufficient for the technical portion. You have the option of sending selected examples of your work or providing direct links to selected publications of your related work, within the published guidelines. Q3: The assumption is that forms 1034, 1034(a) and/or 1035(a) are only to be completed if/when requesting payment for services rendered. Is the correct?A3: This is correct. Q4: The assumption is that it will be the responsibility of MMS to compile the board of 3-6 members based on the proposals received. Is this correct?A4: This is correct. MMS expects to receive proposals of individuals, not proposals for compiling the complete board. Q5a: Offeror Representations and Certifications: What format is acceptable for submission of representations and certifications the annual electronic version has not been completed?A5a: You can either hand write or type the information into the form. Or, visit http://acquisition.gov/far/current/html/52_212_213.html and copy and paste the information from 52.212-3 into a word document and fill out the information. Q5b: There is a lower-case "o" in many of the clauses, what does this "o" mean?A5b: The lower-case "o" is indicative of a "checkbox". For example: in paragraph (b)(4) "Type of Organization" there are several selections with an "o" just before the type of organization. You can check this box manually or place an "X" where the "o" is for the appropriate type of organization. Also, in paragraph (c)(1) the sentence is "The offeror represents as a part of its offer that is "o" is or "o" is not a small business concern". The company should check the "o" or mark with an X the appropriate selection whether or not they are a small business.
