- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. RFQ VA-263-08-RQ-0384 is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-26. The applicable NAICS code is 339112. The requirements of this solicitation are a quantity of one each of the following items: (1) Invisio Smith Percutaneous Nephroscope, Part Number IPN-2505; (2) Obturator, 26 Fr, Smith Percutaneous Nephroscope, Part Number IPN-2526B; (3) Sheath, 26 Fr, Smith Percutaneous Nephroscope, Part Number IPN-2526-S; (4) Cyberwand USL System, P/N: CW-USLS; (5) Cyberwand USL Transducer, Part Number CW-USLT; (6) CW Sterile Transducer Tray, Part Number CW-USLTT; and (7) Cyberwand Rena Bladder Probe, Part Number CW-USLRBP. All quotes submitted in response to this RFQ must remain firm through September 25th, 2008. Delivery required 30 days after receipt of order. Shipping shall be FOB Destination to: Nebraska-Western Iowa Health Care System - Warehouse, 4101 Woolworth Avenue, Omaha, NE 68105. The closing date and time of this RFQ is September 3rd, 2008, 4:00 PM CST. Quotes shall be submitted on company letterhead and signed by an authorized company representative.
