- PRESOLICITATION NOTICE: Alaska VA Healthcare System and Regional Office will issue Request for Quote (RFQ) VA-260-08-RQ-0397 for the Department of Veterans Affairs (VA), to procure Durable Medical Equipment Service, within and from the state of Alaska, to provide all storage, delivery, labor, materials, equipment, tools, parts and supervision necessary to pick-up, clean, maintain, store and deliver Government owned Durable Medical Equipment (DME), as specified within and requested by the Prosthetic and Sensory Aids Service (PSAS), COTR or the Contracting Officer for the Alaska VA Healthcare System and Regional Office (AVAHSRO), 2925 DeBarr Road, Anchorage, AK 99508, for a period not to exceed 5 years, with beginning October 1, 2008, with the right to cancel after September 30, 2009 after 30 days written notice. Quotes will be considered from all offerors who are currently established and regularly providing these services, who are financially responsible and must provide documentation that all personnel are qualified to perform these services with quote submission. All offerors must meet or exceed the current Joint Commissioned Accreditation of Healthcare Organizations (JCAHO) standards or other comparable accrediting body and provide substantiating proof to that fact within six months of award. Solicitation No VA-260-08-RQ-0397 will be electronically issued in commercial item format in accordance with FAR Part 12 and FAR part 13 on approximately 2 September 2008 with a closing date of 23 September.
