- The Centers for Disease Control and Prevention (CDC) intends to issue sole source firm-fixed price purchase order for a maintenance agreement to Roche Diagnostics Corporation, 9115 Hague Road, Indianapolis, Indiana 46250-0457, for the following item(s) and intends to set-aside for a small business concern. Delivery to: Attn: Daphne Hale, 1600 Clifton Road, NE, Atlanta, Georgia. The maintenance agreement will be for a period of one (1) year. Line Item 0001 MagNa Pure System (LCPG1061); Lightcycler 480 (1066); MagNa Pure System (MPCB0722); MagNa Pure System (MPCB0376); and MagNa Pure System (MPCB0528). The agreement will provide unlimited on-site emergency service Monday through Friday, 8:00 a.m. – 5:00 p.m. (local time), on-site response time within 24 hours, Technical Support Call Center providing 24 hour technical telephone support, maximum of one (1) preventive maintenance visit by Roche Diagnostics representive and necessary warranted parts for all machines. Repair and maintenance must be supplied directly by Roche Diagnostics Corporation to avoid invalidation of equipment warranties and/or service agreement coverages. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS PREPARED IN ACCORDANCE WITH THE FORMAT IN SUBPART 12.6, AS SUPPLEMENTED WITH ADDITIONAL INFORMATION INCLUDED IN THIS NOTICE. THIS ANNOUNCEMENT CONSTITUTES THE SOLICITATION; A SEPERATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The following provision(s) apply to this acquisition: 52.212-1 Instructions to Offerors? Commercial Items (MAR 2000) applies to this acquisition; 52.223-13 Certification of Toxic Chemical Release Reporting; 52.223-14 Toxic Chemical Release Reporting; 52.232-1 Payments; 52.233-3 Protest After Award; 52.201-1 Definitions; 52.203-5 Covenant Against Contingent Fees; 52.203-7 Anti-Kickback Procedures; 52.204-7 Central Contractor Registration; Contractor must be CCR Registered prior to submitting bid; The clause 52.212-4, Contract Terms & Conditions - Commercial Items (MAY 1999) applies to this acquisition. The following FAR Clauses are hereby added to this clause: 52.247-35 F.O.B.
