- This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Request for Quote is W9124B-08-T-0129 issued as request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-16 dated 22 March 07. The associated NAICS code is 813110. Contractor shall provide the following specific tasks: ITEM DESCRIPTION AND QUANTITY FOR FIRM FIXED PRICE (FFP) CLIN001: QTY: 1,040 EA The Contractor mutually agrees to perform one or more of the following services and be paid the applicable rate for the services after satisfactory rendition. It is further agreed that the Contractor will perform to the best of his/her ability, using due diligence at all times to develop a reasonably satisfactory level of excellence in the service performed, consistent with his/her capabilities and skills. In a practical sense, the contractor will receive clients for appointments. Assist the Family Life Chaplain in appropriately administering and scoring testing. Assist Family Life Chaplain with triage of clients. Maintain appointment book. Answer office phones and direct calls. Assist with case management and referral coordination. CLIN 0002: QTY: 520 EA Record keeping, maintain sensitive files and sensitive information, data input. Provide correspondence, regarding routine and sensitive issues, receive, inspect, organize, store and log shipments, and supplies. CLIN 0003: QTY: 416 EA Assist in designing implementing special programs. Assist in the preparation and coordination of materials and equipment for training, retreats, and other events. Coordinate advertising and registration for Family Life Center events. Recruit volunteers to support events as needed. CLIN 0004: QTY: 52 EA Meet with the Family Life Chaplain one hour each week to review the operation of the Family Life Center. CLIN 0005: QTY: 52 EA Prepare and post announcements of Family Life Center events (Postmaster, High Desert Warrior, bulletins board). Submit all publicity announcements to the Installation Chaplain for approval before release to the public. The Contractor will either personally perform the services described in this Statement of Work (SOW). This Contract is payable only to the Contractor. The Contractor will be paid only for the services performed. Requested Period of Performance: 01 Oct 08- 30 Sep 09.
