Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The reference number is F3Z4538171A003; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-26 and Defense DCN 20080812. This acquisition will be unrestricted; the North American Industry Classification System (NAICS) code is 811219. The business size standard is $7M. The Standard Industrial Classification (SIC) is 7699. The Federal Supply Code (FSC) is J065. This request for quotation consists of the following service: Item 0001: Qty 12 months, Non-personal services to provide preventative maintenance including intervening service calls and parts for government owned Varian Linear Accelerator and Varian Multi-Leaf Collimator IAW the performance work statement to include: Varian Linear Accelerator, Clinac 2100C, Serial #270022022, Index 13688 and Varian Multi-Leaf Collimator, MLC-80, Serial #541255. Item 0002: Qty 1 Year. Parts shall be itemized and billed monthly per system. Once annual not to exceed estimate has been reached, contractor shall be responsible for that system's parts for the remainder of the performance period at no additional cost to the government. Item 0003: Qty 1 Hour, Rate for services performed outside PPM (0800 to 2000) but prior to 2400 Monday through Saturday, excluding Federal Legal Holidays IAW the PWS. Estimated amount shall not be exceeded without prior written approval of the Contracting Officer. Line item will be funded when required. Item 0004: Qty 1 Hour, Rate for services performed on Sundays, Federal Legal Holidays and after 2400 (midnight), but before 0800, Monday through Saturday IAW the PWS. The estimated amount for this service shall not be exceeded without prior written approval of the Contracting Officer. Line item will be funded when required. Services shall be performed at: David Grant Medical Center 101 Bodin Circle, Bldg 777, Travis AFB, California 94535. Period of performance: 1 October 2007 through 30 September 2008. Contractors are asked to submit their tax identification number with their proposal. Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer. The following provisions and/or clauses apply to this acquisition. Offerors must comply with all instructions contained in FAR 52.204-7 Central Contractor Registration. FAR 52.204-9 Personal Identity Verification of Contractor Personnel. FAR 52.209-6 Protecting the Government's Interest When Subcontracting. FAR 52.212-1 Instructions to Offerors -- Commercial Items. FAR 52.212-2 Evaluation - Commercial Items; paragraph (a) to this provision is completed as follows: price and technical capability of the item offered to meet the Government requirement. Technical capability to meet the statement of work is approximately equal to price in terms of relative importance to the evaluation criteria. Quotation will be evaluated by the ordering activity to conclude that the order represents the best value and results in the lowest overall cost alternative to meet the Government's needs.to determine the best value to the government.
