- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. The announcement constitutes the only solicitation, proposals are being requested and a written solicitation will not be issued. The RFQ number is CCHCF-AB-08-0005. The NAICS code for this action is 238220. This acquisition will be set-aside utilizing any of the Small Business Administration (SBA) Programs. The Procurement will be conducted using Source Selection Procedures, base on Best Value. In Determining Best Value, the following factors will be considered: Price, Past Performance, and Whom the Government determines capable to accomplish the necessary work in the manner most advantages to the Government. The resultant contract will be Firm Fixed Price Contract. REQUEST FOR QUOTES SHALL SHOW COST BREAK DOWN FOR ALL WORK AND MATERIALS TO BE PERFORMED. COST SHALL BE INCLUSIVE OF ALL MATERIALS, PARTS, LABOR, TRAVEL AND TRANSPORTATION. Quotes are being solicited for the following items. CLIN 1, Carrier Brand, Comfort 80, 58DLA090-1-14, 88 BTU Furnace. Upflow, 71,000 rated heating Output, 1385/1155 selectable cooling. Quantity is 40 EA CLIN 2, Carrier Brand, Comfort 80, 58DLA-110-1-12. 110,00 BTU Furnace, downflow. Quantity is 4 EA CLIN 3. REMOVAL & INSTALLATION. Scope of WorK; a. Remove old existing furnaces, transitional ducts, exhaust vent pipes, thermostat from the Government Housing Compound and transport them to Facility Management Yard at Chinle Hospital, Chinle, AZ b. Install new furnaces with transitional ducts, exhaust vent pipes and new heat only thermostat. c. Perform unit start-up, test runs, set gas pressures, safety test and put in operation. d. Safety inspection on the double wall B vents, gas flex, gas cock, gas leak test, (only if it needs replacement or needs replacement, or needs correction) Contractor will do the correction. e. Contractor shall provide one year workmanship warranty.
