Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W58CFB82314244 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. This acquisition is set aside for Small Business under NAICS code 334510, Size Standard 500. FAR provisions and clauses in this solicitation may be viewed at http://www.arnet.gov/far/. DFARS clauses may be viewed at http://www.acq.osd.mil/dpap/dars/dfarspgi/current/index.html. The requirements in this solicitation: 0001: PRIMUS RS without tools, BTE Technologies #50024011, 1 each; 0002: Sports Pkg, BTE Technologies #50030005, 1 each; 0003: Manual, PRIMUS RS service, 2 ea BTE Technologies #40034009; 0004: Manual, PRIMUS RS Operators, 1 ea, BTE TEchnologies #40039000. BRAND NAME OR EQUAL BTE TECHNOLOGIES. Vendors offering other than the brand name product shall meet or exceed the following Salient Characteristics: 1) machine must simulate several different activities such as driving, shoveling, turning, twisting, pulling. 2) Machine must show the work and power put out by each activity. 3) Shall have capability to increase or decrease effort depending on the status of the patient. 4) Information must be stored and displayed at each treatment session. 5) Support a variety of different diagnoses, as a minimum: stroke, TBI, amputee. Delivery date is 30 Sep 08. Place of delivery and acceptance is General Leonard Wood Army Hospital, Fort Leonard Wood, MO. FOB destination is preferred. FAR provision 52.212-1, Instructions to Offerors Commercial, applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to award. Vendors shall comply with FAR 52.211-6 Brand Name or Equal. Clause 52.211-6 and other clauses throughout this solicitation can be viewed by accessing website http://farsite.hill.af.mil/. Offers will be evaluated according to 52.212-2.
