Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This combined synopsis/solicitation is issued as a request for proposal (RFP). The reference/solicitation number is VA-250-08-RP-0374. Submit written offers. Oral offers will not be accepted. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) FAC 2005-26. The full text of the Federal Acquisition Regulations (FAR) and Veterans Affairs Acquisition Regulations supplement (VAAR) can be accessed on the Internet at http://www.arnet.gov/far (FAR) and http://vaww.appc1.va.gov/oamm/vaar (VAAR). THIS PROCUREMENT IS SET ASIDE FOR 100% SERVICE-DISABLED VETERAN-OWNED SMALL BUSINESS (SDVOSB). All firms or individuals responding must be registered with the Central Contractor Registration (CCR) (www.bpn.gov) and VetBiz (www.vetbiz.gov). The applicable NAICS is 561730. Size standard for small business is $6.5 million. It is highly recommended that you complete the solicitation mailing list on the web site before downloading the combined synopsis/solicitation. Any amendments issued to this combined synopsis/solicitation will be available ONLY at the website. Offerors are reminded that they are responsible for obtaining all amendments from the website.SCOPE OF WORK The Veterans Administration Ambulatory Care Center (ACC) is soliciting bids for the procurement of Exterior Lawn Maintenance Service at 420 N. James Road, Whitehall, Ohio. The landscape contractor shall provide all personnel, equipment, tools, qualified supervision, incidental costs, including transportation and lodging and other items and services required to complete all landscape maintenance work to include general lawn care maintenance; turf maintenance to include general cleanup and waste removal, mowing, edging, fertilizing, lawn weed control and insect and disease control monitoring; care of trees, shrubs and ground cover to include pruning and restraining or restricting the growth of vines, and snow plowing and removal. Spring cleanup to include trash removal and cleanup, fertilizing and mulching of plant beds; Fall clean up to include leaf removal. All chemical applications shall be performed in accordance with current county, state, and federal laws, utilizing EPA registered materials and methods of application. These applications shall be performed under the supervision of a Licensed Certified applicator. All services to be performed by fully licensed and accredited personnel. The businesses must, be an established, financially responsible, and have the necessary equipment and personnel to furnish lawn maintenance service as described in the solicitation. DESCRIPTION OF WORK ***See Attached Work Statement for Grounds Services*** Any additional services are to be used to satisfy the Government's short-term nonrecurring needs for grounds maintenance not covered under the (basic) services stated above. Should a continuing need for additional services arise, a contract modification will be negotiated pursuant to the "Changes" clause contained in this contract under Far 52.212-4. Orders for additional services are to be placed in writing by the Contracting Officer or the COTR based on a firm-fixed negotiated prices. The Government reserves the right to unilaterally modify the contract to increase or decrease the "Not to Exceed" amount for Additional Services established in the purchase order. RESPONSIBILITIES OF THE CONTRACTOR The Contractor shall provide all labor, management, supervision, equipment, services and supplies necessary to provide the required services to maintain all trees, shrubs, ground covers, flowers and lawn areas in a healthy, attractive, and safe condition. The Contractor shall also remove and replace all dead or diseased trees and shrubbery, and re-sod/replace grass as needed and/or determined by the COTR. Services shall also include the proper clean-up and off-site disposal of all clippings, pruning, etc., generated in the process. All work shall be performed in a professional manner within the standards of the industry using quality equipment, methods and materials. The contractor shall provide the COTR in advance an anticipated monthly schedule of work that needs to be performed. COMPETENCY OF CONTRACTOR PERSONNEL All services under this contract shall be performed by skilled personnel directly employed and supervised by the contractor. No foreign materials or non-United States citizens shall be utilized to perform any work required by this contract. Should contractor personnel changes occur during the term of the contract, the Government requires services to be of a sustained superior performance. LICENSES, PERMITS AND INSURANCE The contractor shall obtain and maintain at his/her own expense all necessary licenses, permits and insurance, and shall conform to all laws, regulations and ordinances applicable to performance of the contract. Upon award, the contractor shall provide to the Contracting Officer proof of insurance coverage. PARKING Contracting personnel shall park in areas as designated by COTR. LIABILITY FOR INJURY OR DAMAGE The contractor shall not be held liable for any injury to Government personnel or damage to Government property arising from performance of contract services, unless such injury or damage is due to the fault or negligence of the contractor. FAILURE TO PERFORM INSPECTIONS AND SUBMIT REPORTS Contractor's failure to perform inspections and submit reports within the timeframe will result in monetary deductions being taken. Any exceptions to this requirement must be approved, in writing, by the Contracting officer or COTR. If verbal approval is given, written approval will follow within five (5) work days. SITE VISIT Pre-bid site visit scheduled for September 10, 2008 @10:00 AM, beginning at the ACC (Walsh Construction Office Trailer). ALL PERSONS INTERESTED IN BIDDING ARE ENCOURAGED TO ATTEND THE SITE VISIT. PERIOD OF PERFORMANCE The Governments intent is to award a base year with four option year for renewal. All work shall be in accordance with all terms, conditions, provisions and the Schedule of this combined/ synopsis/solicitation for the date of award, October 1, 2008 through September 30, 2009 base year, and four subsequent renewal periods, from date of award through the following FY. Base Period: 10/01/2008 - 09/30/2009 Option Year I: 10/ 01/2009 - 09/30/2010 Option Year II: 10/01/2010 -09/30/2011 Option Year III: 10/01/2011 -09/30/2012 Option Year IV: 10/01/2012 -09/30/2013 Evaluation will be Lowest Price- Technically Acceptable. Offerors are required to return a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications- Commercial Items, with its offer.
