- Courier Service VA-251-08-RQ-0278: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. The North American Industry Classification System Code is 492110, Courier Services. The small business size standard is 1500 employees. This procurement is a total set-aside for small service-disabled veteran-owned business concerns in accordance with IL-07-08.1 VA Notice of Total Veteran-Owned Small Business Set-Aside. Offerors are encouraged to submit quotes based on the work requirements specified herein for a base period from date of award (approximately October 1, 2008) through September 30, 2009, with four pre-priced option years from October 1, 2009 through Sept 30, 2013. Proposed rates should include all costs associated with Request For Quote VA-251-08-RQ-0278 as follows: Work Statement C.0. SCOPE: The successful offeror shall provide all vehicles, gasoline, insurance, personnel, and directional materials to necessary to accomplish regularly scheduled courier services for and between the Department of Veterans Affairs Medical Center (VAMC), Battle Creek, Michigan and the Department of Veterans Affairs (VA) Community-Based Outpatient Clinic (CBOC), Benton Harbor, Michigan and between Department of Veterans Affairs Medical Center (VAMC), Battle Creek, Michigan and the Department of Veterans Affairs (VA) Community-Based Outpatient Clinic (CBOC), Lansing, Michigan.. Services shall be provided Monday through Friday, excluding government holidays. C.1. 0 Schedule/Locations/Items: C.1.1. Work Hours - Pick up at the Department of Veterans Affairs Community-Based Outpatient Clinic, 115 West Main Street, Benton Harbor, MI 49022 between 3:30 p.m. and 4:00 p.m. and deliver shall be made no later than 6:00 p.m. to the VAMC, 5500 Armstrong Road, Battle Creek, Monday through Friday, excluding National Holidays. Please list pricing for route separately. Pick up at the Department of Veterans Affairs Community-Based Outpatient Clinic, 2025 South Washington, Lansing, Michigan 48910, between 3:30 p.m. and 4:00 p.m. and deliver shall be made no later than 6:00 p.m. to the VAMC, 5500 Armstrong Road, Battle Creek, Monday through Friday, excluding National Holidays. Please list pricing route separately. C.1.1.1. National Holidays: The ten (10) holiday observed by the Government are: New Years Day, President's Day, Martin Luther King's Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, veterans Day, Thanksgiving Day, Christmas Day and any other day specifically declared by the President of the United States to be a national holiday. C.1.1.2. Insurance - The Contractor shall have the sufficient insurance on contractor owned vehicles and personnel. It should be noted that blood substances will be one of the items transported by the contractor. C.1.1.3. Inclement Weather Provision - The Contractor shall contact the CBOC coordinator or designee when the Contractor feels that weather indicates adjustment or cancellation of scheduled pick up/deliveries. Contact information will be provided upon award. C.1.1.4. If the contractor fails to furnish the required services within the time frames established, the Department of Veterans Affairs reserves the right to obtain the service from another source and to charge the contractor with any excess cost which may result therefrom. C.1.1.5. Contact - The CBOC coordinator or designee shall be the main point of contact. Contact information will be provided upon award. C.1.2 Description of Items: All items that contain veteran identification will be transported in a locked container. The contractor is responsible for obtaining the container and providing 2 keys for VA Staff to open and lock the container. (One key to be kept at the CBOC and other key to be kept at the drop location in Battle Creek.) Items that may be transported include, but are not limited to: C.2.1. Blood samples - to be transported in a locked container specifically designed for blood transportation. (Attachment #1 The Veterans Affairs medical Center Battle Creek & CBOC's Blood Borne Pathogens Exposure Control Plan) C.2.3. Medical Records - to be transported in a locked container. C.2.4. Mail - to be transported in a locked container. C.2.5. The number of items being transported will be counted prior to being placed into the locked container and recorded on a spread sheet. When the items are delivered they will again be counted and recorded. These logs will be submitted to the COTR on a monthly basis. See attached spread sheet example. Attachment #2 C.3. QUALIFICATIONS: C.3.1.Offers will be considered only from those offerors who are regularly established in the business called for and who are financially responsible and have the necessary equipment and personnel to furnish services in the volume required for all the items under this contract. Successful offeror shall meet all requirements of Federal, State or City codes regarding operations of this type of service. C.3.2.Each offeror shall submit, with the offer, a letter in duplicate fully describing the make of vehicles, model and year which he/she agrees to furnish under this proposal including the location and telephone numbers of his/her establishment where calls are received and vehicles are immediately available for dispatch. C.3.3.The right is reserved to thoroughly inspect and investigate the establishment, facilities, business reputation and other qualifications of any bidder and to reject any bid, irrespective of price, that shall be administratively determined lacking in any of the essentials necessary to assure acceptable standards of performance. C. 4.0. DRIVERS/ATTENDANTS: C.4.1.A record of each employee as to character and physical capabilities of performing the duties of a courier shall be maintained and made available for inspection upon request. C.4.2.All drivers shall have attained legal age (the age of 21 years) and shall maintain neat, clean and professional appearance. Drivers shall be licensed in accordance with the State of Michigan, to include any special licensure required for the transporting of hazardous waste. C.4.3.Drivers shall not make non-emergent stops, for any reason, during the transport of items under this contract. C.4.4.Contracted employees will have Special Agreement Check (SAC) background checks and finger printing completed by the Battle Creek VAMC. They will be issued VA Contractor ID badges that they will be required to wear when in VA facilities. C.4.5.Drivers will complete annual VA National Privacy Training and submit proof of completion to COTR. See attachment #3 for options for completion. C.5.0. VEHICLES: Vehicles used in the performance of this contract shall be licensed and meet all minimum vehicle requirements as mandated by the State of Michigan, to include a Hazardous Waste Certificate if required. C.6.O. LIMITATIONS: Under no circumstances shall any person other than the driver ride in a vehicle or handle items being transported. C.7.0. SECURITY: While on VA grounds, contractor shall secure vehicles when left unattended. If vehicle must be left unattended with motor running, a second set of keys will be necessary, so that vehicle can be securely locked. Offerors are required to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, (JUN 2008), with its offer.
