Description
(i) This is a combined/synopsis solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. (ii) This is solicitation number FDA-SOL-08-1044702 and this solicitation is being issued as a Request for Quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-12. This requirement is open to full and open competition. (iv) The NAICS code is 541519. The size standard is $25M for small business concerns. (v) The following is a list of contract line item numbers(CLIN), items, quantities, and units of measure: CLIN 0001 Recovery of Data from failed RAID array, 1 Job (vi) DESCRIPTION OF REQUIREMENTS: STATEMENT OF WORK: The U.S. Food and Drug Administration requires data recovery services to recover data from a failed RAID array previously attached to the FDA CBER file server. The failed 1.7 TB RAID5 logical drive was built with a Dell PERC card attached to a Dell disk shelve housing 14 physical drives. It is critical to the work of the FDA’s Center for Biologics Evaluation and Research that as much data as possible be recovered from this failed drive set. The U.S. Food and Drug Administration requires 1) That the vendor provide and deliver to the FDA the necessary shipping materials and postage to package and ship the 14 hard drives that make up the failed RAID set in a safe fashion from the FDA to their facilities for evaluation and reconstruction, 2) That the vendor performs a diagnostic evaluation of the failed RAID 5 set upon receipt to indicate the vendor’s ability to recover data from the failed drive set, 3) That the vendor report once daily the status of the diagnostic evaluation to the FDA Technical Point of Contact between 9:30 AM and 5PM Monday through Friday via email or phone, 4) That the vendor report the final outcome of the diagnostic evaluation of the failed RAID 5 set to the FDA’s Technical Point of Contact, 5) The report should indicate at a minimum the overall health of the drive set, the percentage of data retrievable, and a list of retrievable files, 6) That the vendor restores all identified recoverable data from the failed RAID 5 set to large external USB attachable drives, 7) That the vendor report once daily the status of the data recovery to the FDA Technical Point of Contact between 9:30 AM and 5PM Monday through Friday via email or phone, 8) That the vendor ships the USB attachable drives to the FDA Technical Point of Contact in a safe and secure fashion to prevent unauthorized access to the data on those drives during handling and transit. The vendor should not expect to get these USB drives back, 9) That the vendor provide the necessary shipping materials and ship the original 14 hard drives back to the FDA Technical Point of Contact in a safe fashion Packing, Shipping Instructions – That the vendor provide the necessary shipping materials to package and ship 14 hard drives in a safe fashion from the FDA to their facilities for evaluation and reconstruction, That the vendor ship the USB attachable drives to the FDA Point of Contact in a safe and secure fashion to prevent unauthorized access to the data on those drives during handling and transit, and that the vendor provide the necessary shipping materials and ship the original 14 hard drives back to the FDA Technical Point of Contact in a safe fashion (vii) Place of Delivery and Technical Point of Contact - Christopher Kiem, 5600 Fishers Lane, Rockville, MD 20850 Room # 17B17, Phone: 301-796-4081 Email: Christopher.kiem@fda.hhs.gov. (ix) The provision at 52.212-1, Evaluation -- Commercial Items (Jan 1999) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will offer the best value to the Government. This is a competitive best value source selection in which the Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price, and other factors considered. Refer to the attached evaluation criteria. (x) Vendors shall complete FAR 52.212-3, Offerors Representations and Certifications - Commercial Items (JUN 2003). If the vendor does not have active representations and certifications registered online they will be disqualified from being considered for award.
