- Tumor Registrar/Cancer Registrar - Needs to be a CTR (Certified Tumor Registrar) "Compiles/Abstracts/Analyzes Data from Records. Obtains information from patient's charts regarding tumor and cancer diagnoses. Maintains cancer information in an appropriate registry. Answers requests for data and prepares routine and special reports when the requests are for data with which the employee is familiar and can be provided in a format established by higher graded employees. The Health Information Management (HIM) Office provides direct supervision for all duties performed in regard to the tumor registry function. Other members in the Oncology Section may also provide guidance and supervision for the duties related to this function. Will participate in routine conference calls with HIM office. "Employee composes responses to routine requests for release of patient information. Employee will maintain a log of all disclosures and make it available upon request to the Health Information Management Office. Employee will work with Health Information Management and ensure they have legal authority to disclose information either by obtaining written consent from Veteran's or validating reporting is allowed under a "Standing Letter". Employee may be required to participate in a Data Transfer Agreement, per VA directive. "Employee Selects and compiles information from medical records and prepares correspondence. Prepares correspondence by retrieving and releasing data following a standardized approach, which meets the requirements of VA Privacy Act and Freedom of Information Act regulations. Typically uses VA approved Release of Information forms and other standard forms or formats for letters. "Maintains tumor registries, performs research. Maintains special registries of disease occurrences. Assists in research. Incumbent works with IRM to ensure that DHCP Oncology software is functioning correctly. Ensures that the data entered into the computer is accurate and complete; data variation will be brought to the attention of the Chairman of the Cancer Committee and IRM. Maintains annual follow-up on all cases. As directed the employee gathers information for research and correspondence. Compiles information in final format for inclusion in registries, or to be provided for research or correspondence. Information is obtained from sources that employee is familiar with and consists of data with which the employee has previously worked. "Communicates with the National Cancer Institute and pharmaceutical companies regarding the acquisition of compassionate use drugs; explains and justifies need for drugs; completes and submits necessary forms in accordance with established procedures, maintains registry to show occurrences of disease by type, extent of illness and survival. Information is gathered from various sources and the employee must use judgment to select accurate information for inclusion. Organizes and maintains a data system which conforms to the standards of tumor registry operations and meets the needs of the users of the registry data. Tumor Registrar: These items pertain to accuracy and timeliness and are requirements for the American College of Surgeons (ACoS) accreditation. 1. All eligible cases are identified (both new cases and those already included in the registry database). 2. 90% of cases are abstracted within six months of the date of first contact. 3. Data accuracy is met for: class of case, primary site, histology, first course of treatment and follow-up information. Benchmark: 90% of cases reviewed are accurate 4. Follow-up rates are maintained for all analytic patients from the cancer registry reference date Benchmark: 80% follow-up rate maintained from registry reference date 5. Follow-up rates are maintained for patients diagnosed within the last five years Benchmark: 90% follow-up rate maintained for all analytic patients diagnosed within the last 5 years 6. Cases for the most recent accession year satisfy the established quality criteria and deadline date specified in each National Cancer Data Base (NCDB) Call for Data Benchmark: 100% accuracy. 7.
