Description
The Department of Veterans Affairs, VA Caribbean Healthcare System (VACHS), San Juan, Puerto Rico, has the requirement to furnish all necessary labor, material, equipment and supervision in the performance of cabling work at St. Thomas, Community Based Outpatient Clinic (CBOC) located at V.I. Medical Foundation, Suite 101, 50 Est. Thomas, US Virgin Islands, for the work listed below. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number VA248-08-RQ-0498. The Request for Quotation (RFQ) is 100% set-aside for small business under NAICS code 238310 with a business size of $14.0M. SAN JUAN VA CARIBBEAN HEALTH CARE SYSTEM (VACHS) NETWORK INFRASTRUCTURE REQUIREMENTS 1. REQUIREMENTS: This job requires the following: MATERIALS DESCRIPTION: 100 SYSTIMAX PATCH CORD 24/4 STRANDED; CAT 6 BLUE 10 FT; 100 SYSTIMAX PATCH CORD 24/4 STRANDED; CAT6 3 Ft; 3 PATCH PANEL 48 PORT CAT6 110 MOD; IPATCH; 1 PATCH PANEL 24 PORT; 2 VERTICAL CABLE MANAGEMENT KIT 824; 6X84 DOBLE SIDE WITH DOOR BLK; 1 RACK RELAY 3"X 7' H 19" 45U DOBLE; SIDE ALUMINUM BLK- RK3-45A; 100 SYSTIMAX JACK CAT6 A/B BLUE; 50 SYSTIMAX FACE PLATE; 125 FEET BELDEN 25 PAIRS CAT5-E; RISER CABLE - 25PUTPCMRCT; 1 BLOQUE 110 SYSTIMAX 200PRS KIT; WITH LEGS- 110AW2-300; a. All new data drops Cat 6 punched on B configuration and RJ45 Ipatch panel. (Systimax panel); b. Test and Certify new data cable category and submit testing results to network management staff; c. All wall jacks will be labeled to represent new closet connection layout, wall jacks will be blue; d. In some cases work will require contractor to work at night and weekends to avoid delay to the project; e. Network management staff will be responsible for the QA on the data cabling installations. Telecom staff will be responsible for the coordination with the contractor; f. Contractor will assure proper supervision of any subcontractor hired to complete the job as required on the existing cabling agreement; g. Contractor should provide a SOW to include amount of days to completed cabling, certification, etc.; h. The VACHS will provide the contractor drawings with voice/data layout so that vendor can provide quotation. 2. REFERENCES: For further reference refer to the following standards: a. All new cable shall be labeled on each end and fully tested. And shall conform to meet the requirements of ICEA Publications S-80-576-1988 (Ref.B1.6) as to size, color and installation; b. A minimum of two pair four (4) unshielded twisted pair 24AWG station within shall be install from each data jack (EIA/TIA 568 Standard "T568A") to the telephone closet and shall be or a type designed to support at least level five data communications (not less than 1000 MHz/1000Mbps.); c. Cable transmission performance specifications shall be in accordance with EIA/TIA standards. Attenuation shall be measured in accordance with EIA fiber optic test procedures EIA/TIA-455-46,-61, or -53. Information transmission capacity shall be measured in accordance with EIA/TIA-455-51 or -30; d. SYSTIMAX Solutions SYSTIMAX Structured Connectivity Solutions, Performance Specifications, (and Addendum) Latest Issue; e. SYSTIMAX Solutions, Components Guide, Latest Issue; f. SYSTIMAX Solutions Design & Installation Guidelines, Latest Issue. 3. CONTRACTOR QUALIFICATIONS: a. Any sub-Contractor, who will assist the Systimax Certify Solution (SCS) contractor in performance of this work, shall have the same training and certification as the SCS contractor; b. All communications cabling used throughout this project shall comply with the requirements as outlined in the National Electric Code (NEC ) Articles 725, 760, 770, and 800 and the appropriate local codes; c. All balanced twisted pair cabling shall bear CMP (Plenum Rated), CM/CMR (Riser Rated) and/or appropriate markings for the environment in which they are installed. 4. CABLE PATHWAY: a. In suspended ceiling and raised floor areas where duct, cable trays or conduit are not available, the Contractor shall bundle, in bundles of 50 or less, horizontal cabling with cable ties snug, but not deforming the cable geometry; b. Cable bundles shall be supported via "J" hooks attached to the existing building structure and framework at a maximum of five (5) foot intervals; c. Plenum rated cable ties shall be used in all appropriate areas; d. The contractor shall adhere to the manufacturers' requirements for bending radius and pulling tension of all data and voice cables; e. Cables shall not be attached to lift out ceiling grid supports or laid directly on the ceiling grid; f. Cables shall not be attached to or supported by fire sprinkler heads or delivery systems or any environmental sensor located in the ceiling air space; g. All of the above requirements are also in the existing telephone maintenance contract. 5. PROGRESS AND COMPLIANCE: VACHS IRM staff will be on site actively participating and monitoring the job progress; nonetheless, it's the Contractor responsibility to perform work in accordance with regulations and applicable codes. Before work area is completed, the IRM staff will inspect the areas to determine its completion. IRM will assign a Project Manager that will monitor overall project compliance and timeframes. This person, in conjunction with the Contractor's Project Manager will determine work area and timeframes for each step of the process. 6. PERFORMANCE PERIOD: The Contractor will complete the work within 10 calendar days from the purchase order effective date. 7. LAYOUT: No later than September 11, 2008 at 11:30am (P.R. Local Time) the Purchasing Agent will provide via e-mail copy of the layout, upon written request from the Company. 8. SAFETY REQUIREMENTS: In performance of this contract the Contractor shall take such service safety precautions as the Contracting Officer or his/her designee may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer or his/her designee will notify the Contractor of any non-compliance with the foregoing provisions and the action to be taken. The Contractor shall, after receipt of such notice, immediately correct the condition to which attention has been directed. Such notice, when served on the Contractor or his representative at the site of work, shall be deemed sufficient for the purpose aforementioned. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue an order stopping all or any part of the work and hold the Contractor in default. 9. Only the Purchasing Agent is authorized to make commitments or issue changes which will affect the price, quantity, quality or terms of this order. The contractor must provide a contact person whom VA may contact during the performance of this contract for prompt action on matters pertaining to the administration of the order. A notice received by this contact person will be considered to be a notice received by the Contractor.
