- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is being issued as a Request for Quotation (RFQ). Submit only written quotations for RFQ N62473-08-Q-2617. This acquisition is Full and Open Competition. The associated NAICS code is 562211 with a $11.5 million size standard. Submit quotes for the following: Line Item 0001, Provide disposal of Medical Waste in accordance with the statement of work for the Base Period (Oct '08-Sep '09). Line Item 0002, Provide disposal of Medical Waste in accordance with the statement of work for Option 1 (Oct '09-Sep '10). Line Item 0003, Provide disposal of Medical Waste in accordance with the statement of work for Option 2 (Oct '10-Sep '11). Line Item 0004, Provide disposal of Medical Waste in accordance with the statement of work for Option 3 (Oct '11-Sep '12). Line Item 0005, Provide disposal of Medical Waste in accordance with the statement of work for Option 4 (Oct '12-Sep '13). The following is the statement of work: Pickup and disposal of medical waste to include autoclave waste such as sharps, laboratory wastes, contaminated disposal material, equipment and instruments, blood, blood products, and body fluids, and incinerator waste such as trace contaminated chemotherapy waste, pathological waste, solidified suction canisters, and non RCRA pharmaceutical waste from Naval Hospital Lemoore, Bldg 937, Back Loading Dock. Pickup as needed but not less than once weekly. Unit rate per pound of pickup disposal = $______per pound for autoclave waste. Unit rate per pound of pickup disposal = $______per pound for incinerator waste. Contractor shall keep a current record of amount of each week's waste pickup/disposal. Contractor shall invoice only for actual weight in pounds picked-up and disposed. Contractor shall comply with all applicable Federal, State and Local Statutes and Regulations. Wage Determination No. 2005-2074, Revision No. 9, dated 6/11/2008, setting forth the minimum prevailing wages, including fringe benefits, is applicable to this contract. Payment shall be made by Government Purchase Card in accordance with FAR 52.232-36, Payment by Third Party. Background information: Naval Hospital Lemoore is a 16-bed facility. Historical volumes for autoclave waste have been: 10,118 lbs from Jan 08 through Jul 08; 17,473 lbs for the entire year of 2007. There are no historical volumes for incinerator waste, but anticipated volumes are up to 360 lbs per year. A firm fixed bid price is required for each of the two items above for each Line Item in order to be considered responsive to the RFP. Award will be made on the lowest unit rate per pound of pickup disposal for autoclave waste for the Base Period, Line Item 0001. Award of and exercise of options are at the discretion of the Government and not guaranteed.
