Description
FISC Norfolk Contracting Department, Groton Office requests responses from qualified sources capable of providing a Civilian Employee Assistance Program (CEAP) for CNRMA naval activities in accordance with the Performance Work Statement. This is a SYNOPSIS for commercial services prepared in accordance with the information in FAR Subpart 12.6, using Simplified Acquisition Procedures for commercial items found at FAR part 13 and Subpart 13.5 (Test Program for Certain Commercial Items) as supplemented with the additional information included in this notice. The government plans to award an Indefinite Delivery Indefinite Quantity Type Purchase Order (IDIQTPO) covering a maximum of 8,000 employees throughtout the United States. Historically, 2,500 employees have been covered mainly throughout the Northeast within these six major geographic locations: (Newport, RI; Groton, CT; Brunswick, ME; Earle, NJ; Uniondale, NY; Philadelphia, PA.This synopsis shall be posted on both the FEDBIZOPPS and NECO (https://www.neco.navy.mil/). The Request for Quote (RFQ) number is N00189-08-T-G110. The NAICS code is 621999 -All Other Miscellaneous Ambulatory Health Care Services, and the small business standard is $ 10 Million. This is a total small business competitive action. Questions regarding technical or contractual issues must be addressed to the Contract Specialist via e-mail or fax ONLY. This RFQ will incorporate provisions and clauses in effect through FAR an d DFARS. It is the vendor's responsibility to be familiar with the applicable clauses and provisions. Lack of registration in the central contractor registration (CCR) database (www.ccr.gov) will make a vendor ineligible for award.
