Description
(i) This is a combined synopsis/solicitation for commercial items prepared in accordance with Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation Number HSCG40-08-Q-42012 applies and this solicitation is issued as a Request for Quotation (RFQ). (iii) The solicitation/synopsis and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-26, effective 12 Jun 2008. (iv) This is a set aside for small business. The NAICS code for this solicitation is 314110 and the small business standard is 500 employees. (v) The United States Coast Guard Engineering Logistics Center has a requirement for the following Contract Line Item Numbers (CLIN’s): ITEM 1. NSN: 7220 01 154 5818, PART NO. 41UT1401-1-REVISION-D, DESCRIPTION: CARPET, 100% WOVEN WOOL, MANUFACTURED TO FEDERAL SPECIFICATION DDD-C-0095A, TYPE II- VELVET, CLASS 2. FLAME SPREAD SHALL MEET ASTM E162 DATED 4/10/02 AND NOT EXCEED THE RADIANT PANAL INDEX OF 25. CARPET SHALL MEET COMMERCE STANDARDS DOC-FF-1-70 AND DOC DD-2-70 COMMONLY KNOWN AS THE "PILL TEST". COLOR: EVEN BLEND OF BLACK, BROWN/AMBER. VENDORS SAMPLES SHALL BE SUBMITTED FOR SELECTION. CUT SIZE: 12 FEET BY 14 FEET. ALL EDGES SHALL BE SEALED AT THE BACKING AND BASE OF THE NAP WITH A SOLVENT FREE ADHESIVE (SEAM SEALER) TO PREVENT UNRAVELING OR DELAMINATION. QTY: 20 SETS. (vi) CONTRACTOR SHALL CONFORM TO THE FOLLOWING PACKAGING, PRESERVATION AND MARKING INSTRUCTIONS: PACKAGING: EACH CARPET SHALL BE INDIVIDUALL WRAPPED IN A WATER PROOF PLASTIC AND PACKED IN A CORRIGATED OR SPRIAL CARDBOARD TUBE/BOX AND SEALED SECURLY FOR CONTAINMENT DURING SHIPPING MARKING:EACH PACKAGE SHALL BE LABLED USING PERMANENT LETTERING AS FOLLOWS: 7220-01-154-5818 CARPET 41UTB1401-1 1 EACH COND CODE: A COAST GUARD ELC MATERIAL (vii) Preparation for Delivery: All material shipped to the U.S. Coast Guard Engineering Logistics Center, Bldg. 88, Receiving Room, 2401 Hawkins Point Road, Baltimore, MD 21226 on a purchase order must be accompanied by an itemized packing list securely attached to the outside of the package. All packing lists shall cite the Purchase Order Number with the package in such a manner as to provide identification. Material shall be packed for shipment in such a manner as to afford adequate protection to the item against corrosion, deterioration, and physical damage during shipment from the supply source to Engineering Logistics Center locations. Package shall also provide adequate protection for warehouse storage and multiple shipments. All packages shall have the purchase order number, stock number (if applicable) and vendor name and part number clearly marked on the exterior of the package. All deliveries are to be made Monday through Friday between the hours of 7:30 AM and 3:30 PM. Failure to following the packaging instructions may result in a company being charged money by the USCG to cover the costs of repackaging the items purchased in any resulting purchase order. Company may also be charged shipping costs to cover the cost of returning the material to the company for repackaging. (viii) Requested delivery date: 03 November 2008. FOB Destination, Delivery to U.S. Coast Guard, Engineering Logistics Center, Baltimore, MD. 21226. (ix) The following FAR clauses apply to this solicitation. Offerors may obtain full text versions of these clauses electronically at http://www.acquisition.gov/far. (x) FAR 52.212-1 Instructions to Offerors-Commercial Items (Jun 2008). Parties responding to this solicitation may submit their offer in accordance with their standard commercial practices (e.g. on company letterhead, formal quote form, etc.) but must include the following information: 1) company’s complete mailing and remittance addresses, 2) discounts for prompt payment if applicable 3) cage code, 4) Dun & Bradstreet number, 5) Taxpayer ID number and 6) Pricing and delivery information. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items with their offer. VENDORS SAMPLES SHALL BE SUBMITTED FOR SELECTION. (xi) FAR 52.212-2 Evaluation-Commercial Items (Jan 1999). – This is a commercial item acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation. The evaluation factors are price and delivery. Award will be a best value decision. Award may be made to other than the lowest price. The Government intends to award on an all or none basis to a responsible offeror whose offer conforms to the solicitation, and provides the Government with the best value. The evaluation and award procedures in FAR 13.106 apply. (xii) FAR 52.212-3. Annual Representations and Certifications – Commercial Items (Jun 2008) - The use of ORCA is mandatory as of January 1, 2005. ORCA is available through Business Partner Network (BPN) at http://www.bpn.gov. Contractors will use ORCA to electronically submit annual representations and certifications (REP & CERTS). If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete paragraphs (b) through (k) of provision FAR 52.212-3 which can be obtained electronically at http://www.arnet.gov/far. (xiii) FAR 52.212-4 Contract Terms and Conditions-Commercial Items (Feb 2007) applies to this acquisition. The following addendums apply: Address for submission of invoices is U.S. Coast Guard YARD, P.O. Box 4122, Chesapeake, VA 23327-4122. Inspection/Acceptance: QA (Quality Assurance) inspection shall be performed by U.S. Coast Guard Quality Assurance Representatives.
