- Motorola EquipmentStatement of Work This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, which is issued as a Request for Quotation (RFQ) Point of Contact is: Stan Hockett, Contract Specialist, 5115 NE 82nd Ave., Suite 203, Vancouver, WA 98662; quotes are being requested and a written solicitation will not be issued. The Government intends to acquire commercial items using FAR Part 12 and the Simplified Acquisition Procedures set forth in FAR Part 13. This procurement is being solicited on an unrestricted basis. However, the offeror shall identify the size of their business. The NAICS Code is 334290 and the small business size standard for this procurement is 750 employees. The offeror shall state in their offer their size status for this procurement. All responsible sources may submit an offer which shall be considered by the agency. Location of where equipment is to be delivered is Portland, OR. Delivery shall be FOB Destination. Proposal must include: solicitation number, proposed delivery schedule, discount/payment terms, warranty duration, taxpayer identification number (TIN), cage code, DUNS Number, CCR number, identification of any special commercial terms, and be signed by an authorized company representative. Offerors shall provide the information required by FAR 52.212-1 (JAN 2008), Instructions to Offerors-Commercial, which is incorporated by reference. If the end product(s) offered is other than domestic end product(s) as defined in the clause entitled "Buy American Act -- Supplies" the offeror shall so state and shall list the country of origin. All contractual and technical questions must be in writing (e-mail or fax) to Stan Hockett not later than 2 pm Pacific time on September 18, 2008. E-mail address is Stan.hockett@va.gov. Technical acceptability will be determined by review of information submitted by the offeror which must provide a description in sufficient detail to show that the product offered meets the Government's requirement. If offerors are not registered via the ORCA website at http://orca.bpn.gov, offerors shall complete and return a copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items with their offer. These representations and certifications will be incorporated by reference in any resultant contract. It is the offeror's responsibility to monitor the internet site for the release of solicitation amendments (if any). DESCRIPTION: The VA has a need for Motorola equipment and training as listed below. The equipment is used for emergency and ordinary communications. The products will be used at various locations on the Portland VA Medical Center campus. 1.SPECIFICATIONS: The VA requires the following Motorola equipment. 1.Model PR8000 Repeater UHF 50 watts duplexer, power supply; one ea. 2.Model PR635 Portable UHF package with key pad, power supply, belt clip, Battery with two year warranty; twenty ea. 3.Model DB201 Antenna; one ea. 4.Model 190098 Mobile base station power supply desk tray coax antenna; 2 ea. 5.Model 5030 ACU tactical package; one ea. 6.Model 5961 DSP-2 radio/4 wire module; three ea. 7.Model 5964 XTL 2500 cable;one ea. 8.Model 2811 universal cable; 2 ea. 9.Model 5041 NXU-2; six ea. 10.Model 0801 null modem cable;1 ea. 11.Model PR8000 repeater UHF 50 watts duplexer, power supply; one ea. 12.Model PR635 portable UHF package with key pad, power supply, belt clip, battery with two year warranty; ten ea. 13.PPC UHF connectors; two ea. 14.Model 88254 antenna mount; one ea. 15.PPC connectors; four ea. 16.Model LMR-400 coax RF cable; 45 ea. 17.Model 88900 four manhours training on system. 18.Four manhours to install and configure JPS unit. 2.KEY FEATURES: Must be Motorola equipment to preserve continuity of use, maintenance, and performance with existing equipment. TRADE INS: None. PERFORMANCE PERIOD: The machine will be needed sixty days after the order is placed with the successful ifferor. WARRANTY: The vendor shall provide warranty information. Unit Each. Number Required - as listed above. Location of requirement: Portland VA Medical Center, 3710 SW US Veterans Hospital Road, Portland, OR 97239. DELIVERY LOCATION: Vancouver VA Medical Center, Vancouver Warehouse, 1601 E. fourth Plain Blvd Vancouver, WA 98661. EVALUATION CRITERIA: Response will be evaluated on the following: Price and price related factor (warranty, repair service locations), Past performance, and Technical. Technical shall include: ease of use, quality of product, ease of assembly, adaptability to service needs, key features, repair history and delivery. Technical and past performance combined are approximately equal to price.
