- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. VA-259-08-RQ-0319 is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-26. This acquisition is unrestricted under NAICS code 334510. The requirements in this solicitation: Pascal Photocoagulator System, as manufactured and distributed by OptiMedica Corporation, consisting of the following components: Pascal Pattern Scanning Laser Technology; fully integrated 532 NM photocoagulator; ergonomic package (slit lamp, electronic touch screen user interface, power controls, wheel chair assessable table); color balanced eye safety filters; 2 pair safety glasses; and operator manual; and laser indirect ophthalmoscope with full one year warranty. All quotes submitted in response to this RFQ must remain firm through October 1, 2008. Delivery required 30 days after receipt of order. Shipping shall be FOB Destination to: VA Eastern Colorado Health Care System - Warehouse, 1055 Clermont Street, Denver, Colorado 80220.
