Description
SYNOPSIS/SOLICITATION: Solicitation Number: RFQ-PO2008910001 Worldwide Travel Insurance (Activity Number 2005-91008B). This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Quotations are to be electronically received (via email) no later than 12:00noon Eastern Time on September 22, 2008 at contract@ustda.gov. This procurement is not set aside for small business because the Contracting Officer has determined that adequate competition does not exist between small businesses for this requirement. The small business size standard is $7M annual sales under NIACS code 524114. Award is limited to U.S. firms or U. S. individuals. Contractor and U. S. subcontractor employees used shall be either U. S. Citizens or non-U. S. citizens lawfully admitted for permanent residence in the United States. The Government contemplates award of a firm fixed price contract. All contractors must be registered in Central Contractor Registration (www.ccr.gov) and On-Line Representations and Certification (ORCA) System in order to receive a contract award. For additional information regarding this solicitation, email contract@ustda.gov, telephone calls will not be accepted. Vendors shall provide accurate Data Universal Numbering System (DUNS)/Tax Identification Number (TIN) information. DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK: Vendor is to provide up to $100,000.00 urgent/emergency health insurance coverage and up to $25,000.00 evacuation insurance coverage for USTDA hosted delegates and participants of USTDA sponsored events. The provided coverage shall be common off-the-shelf services provided under similar commercial services and similar requirements. The length of coverage duration shall depend on the event or duration of travel event. USTDA does not coordinate most events but contracts out coordination/event services as well as delegation selection and delegation logistics coordination. The Contractor under this contract shall be required to accept lists from USTDA Contractors only after USTDA Contracts Personnel approve the list. Payment of insurance shall be done in arrears after each requirement is performed. It is anticipated that most delegates will require coverage for at least one week therefore weekly rates would be most advantageous. DELIVERIES OR PERFORMANCE: The performance of this contract shall start on October 1, 2008. The Contractor shall submit all deliverables under this Contract to the Contracting Officer's Technical Representative (COTR). The Period of Performance for this Contract shall be October 1, 2008 through September 30, 2009 with up to and including two one-year option periods. SPECIAL CONTRACT REQUIREMENTS: This solicitation includes the following additional contract requirements and terms and conditions- CONTRACT CLAUSES: The Solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-26, June 12, 2008. FAR Clause 52.214-4, Contract Terms and Conditions - Commercial Items and FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes and Executive Orders - Commercial Items are incorporated. INSTRUCTIONS/CONDITIONS AND NOTICES TO OFFERORS OR RESPONDENTS: The Provision at Federal Acquisition Regulation 52.212-1, Instructions to Offerors-Commercial (available at www.ustda.gov under DM Solicitation forms), applies to this acquisition with the following addition: The contractor shall submit an email with vendor, name, point of contact information (inclusive telephone number & email address) and coverage information in layman terms for delegations covered under the policy. Price quotation shall state price per unit (day, week) and unit price for the base performance period and two option periods (years). Vendors are reminded to prepare FAR Provision 52.212-3, Offeror Representations and Certifications – Commercial Items as per the instructions in the provision. EVALUATION FACTORS FOR AWARD: FAR 52.212-2, Evaluation, Commercial Items (available at www.acquisition.gov under FAR), is being used. Price and technical factors shall be evaluated equally.
