- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Request for Quote (RFQ) CMTDC0800041. (iii). The RFQ document and incorporated provisions and clauses are those in effect through FAC 2005-08 and all of the requirement stated in FAC 2005-08. (iv) The resultant contract will be awarded as a single award contract. The NAICS code is 238210, Electrical Contracting and Other Wiring Installation Contractors. (v) Provide a price for the following CLINs. CLIN 0001; Labor, Non-personal for Services, for Electrical Services at DISA CONUS, Bldg 3189, 604 Tyler St. Scott AFB, IL 62225 in accordance with the attached Statement of Work. Period of performance is not to exceed 60 days from date of award, firm fixed price type contract. Method of Payment: Government Credit Card (vi) The work consists providing all labor, tools, equipment, and supervision for the purpose of installing multi electrical circuits and outlets in DISA CONUS, Bldg 3189, 604 Tyler Street, Scott AFB, IL 62225-5421. The outlets will also remove any electrical tripping hazards that have been written up by base safety. The contractor shall ensure all circuits/outlets are installed under the national electric code standards. Necessary capabilities include: electrical, mechanical, engineering, design, and carpentry requirements for installation of the following outlets. Project management and supervision of employees during work. All project employees must be licensed electricians and have experience working in and around mission critical environments or have done electric work in BLDG 3189 before. Start-up /Testing of each outlet is required before DISA CONUS will accept the work. Specific Tasks 6.1 Method of procedure A complete method of procedure will be set up with DISA CONUS before commencement of work. 6.2 The installation of each outlet will be installed using the national electric code standards 6.3 Power for outlets 1200 AMP Service: a) two (2) 110 volt duplex outlets in E-wing north wall b) Ten (10) 110 volt quad outlets in Room72B (North - Comm Room) c) Ten (10) 110 volt quad outlets in Room 33 sub floor area. d) Two (2) 110 volt duplex outlets in Room 65 (Commander's Conf Room-Side B). e) one (1) 110 volt duplex outlets in Room 15B (SCIF). 1600 AMP Service: a) Two (2) under the floor 110 volt duplex outlets in Room 15 (Plum Room-underneath table) b) Three (3) 110 volt duplex outlets in Room 42A (Security Office) c) Three (3) 110 duplex outlets in Room 43 (Property Management) d) Four (4) 110 duplex outlets in I-Wing conference room. e) Six (6) 220 volt duplex outlets in the 2-B-Wing TSC portable A/C units 3-D-Wing, Room 33 for portable A/C units 1-Rm 72 B Jwing for portable A/C units f) Two (2) 110 volt duplex outlets in D wing for printers g) Two (2) 110 volt duplex outlets in C-wing 6.4 Provide a written test report 6.4.1 A start up and inspection on each outlet/circuit: 6.4.2 A written report will be done to show as a minimum the correct volt and ampage draw for each outlet. 6.4.3 Make and install new labels for each breaker box that circuits were ran from 6.5 Clean up any area that the contractor has worked in. Clean up all mech room removing trash and left over material Clean up attic-same as the mech rooms vii) Place of Performance: DISA CONUS, Bldg 3189, 604 Tyler Street, Scott AFB, IL 62225-5421. Contact DISA/CONUS Facility Manager, Ralph Mullins at 618-220-8889 or Ralph.Mullins@disa.mil to arrange a site survey. Site survey is at the expense of the contractor. (viii) FAR 52.212-1, Instructions to Offerors-Commercial Items applies to this acquisition. (ix) FAR 52.212-2, Evaluation -- Commercial Items applies this acquisition. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer confirming to the solicitation will be most advantageous to the Government, price and other factors considered. This will be a Least Price, Technically Acceptable (LPTA) award. a. Award all or none basis - The government intends to award one to the vendor whose proposal is identified as Least Price, Technically Acceptable (LPTA) solution. b. The government reserves the right to not award a contract as a result of this competition, if in the opinion of the Contracting Officer, none of the submitted quotations would provide satisfactory performance/product. (x) FAR 52.212-3 applies; vendors are to download and complete these reps and certs from the FAR site referenced below and submit with offer. (xi) 52.212-4 Contract Terms and Conditions-Commercial Items, applies to this acquisistion (xii) 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Coml. Items applies to this acquisition. The following DFARS clauses apply to this acquisition: 252.204-7000, 252.204-7003, 252.212-7000, 252.212-7001, 252.225-7001, 252.225-7002, 252.227-7015, 252.232-7003, 52.252-1, 52.252-2, 52.252-5, 52.252-6, 52.253-1, (access to clauses can be made via http://farsite.hill.af.mil/). 52.233-2-Service of Protest (Aug. 1996). (a) Protests, as defined in section 33.101 of the Federal Acquisition Regulation, that are filed directly with an agency, and copies of any protests that are filed with the General Accounting Office (GAO), shall be served on the Contracting Officer, DISA/DITCO, 2300 East Drive, Scott AFB IL 62225. (b) The copy of any protest shall be received in the office designated above within one day of filing a protest with the GAO.(End of Provision). 52.209-9000 ORGANIZATIONAL AND CONSULTANT CONFLICTS OF INTEREST (OCCI) DEC 2005) (a) An offeror shall identify in its proposal, quote, bid or any resulting contract, any potential or actual Organizational and Consultant Conflicts of Interest (OCCI) as described in FAR Subpart 9.5. This includes actual or potential conflicts of interests of proposed subcontractors. If an offeror identifies in its proposal, quote, bid or any resulting contract, a potential or actual conflict of interests the offeror shall submit an Organizational and Consultant Conflicts of Interest Plan (OCCIP) to the contracting officer. The OCCIP shall describe how the offeror addresses potential or actual conflicts of interest and identify how they will avoid, neutralize, or mitigate present or future conflicts of interest. (b) Offerors must consider whether their involvement and participation raises any OCCI issues, especially in the following areas when: (1) Providing systems engineering and technical direction. (2) Preparing specifications or work statements and/or objectives. (3) Providing evaluation services. (4) Obtaining access to proprietary information. (c) If a prime contractor or subcontractor breaches any of the OCCI restrictions, or does not disclose or misrepresents any relevant facts concerning its conflict of interest, the government may take appropriate action, including terminating the contract, in additional to any remedies that may be otherwise permitted by the contract or operation of law. The contract type will be firm-fixed price. All unit prices are firm-fixed price. (xiii) N/A (xiv) There is no DPAS rating.
