Description
The WJB Dorn Veterans Affairs Medical Center (VAMC), Columbia, South Carolina has a requirement for Pharmacy Services to fill an estimated 850 prescriptions on an annual basis. Services will be performed at the Department of Veterans Affairs Community Based Outpatient Clinic (CBOC) at 1330 Boiling Springs Hwy, Spartanburg, South Carolina 29303. Contractors proposing to submit a proposal under this requirement shall be located within a two (2) mile radius within or adjacent to the premises of the Department of Veterans Affairs Community Based Outpatient Clinic (CBOC). The two (2) mile radius factor is established to support the medical needs of the Veteran Patients receiving pharmacy prescriptions services under this requirement. The period of performance begins 9/30/08 through 9/29/09. This is requirement is being solicited as an Unrestricted Solicitation to afford ALL responsible business concerns small and large an opportunity to submit a proposal to be evaluated. The Government intends to evaluate proposals and make award without discussions in accordance with FAR Clause 15.306. All bidders submitting a proposal are required to be registered in the Central Contractor Registration (CCR) database prior to final award. A written solicitation will be posted on or around 9/22/08. Services to be performed: Services shall include the dispensing of pharmaceuticals as prescribed by clinic staff. Primarily, new prescriptions will be filled. In the rare instances, where refills may be necessary, the Pharmacy filling the prescription will be provided a pre-authorization by the Clinic Manager. Unless otherwise pre-authorized, prescription fills will be limited to a maximum of a ten day supply. Prescriptions filled under this contract will be to the extent permitted by law, the least expensive drug product the pharmacy has in stock generic drugs will be dispensed whenever possible. A product is not considered a generic equivalent unless the strength quantity and dosage are the same as the brand name. The generic name must be displayed on all labels for all medication. The authorized pharmacy will provide the date of filling, quantity provided (if other that the prescribed quantity), prescription number and name or initials of the responsible pharmacist. The contractor shall be paid only for those prescriptions that are approved by the VA Clinic.
