REQUEST FOR QUOTE Snow Removal Petersburg Supervisor’s Office and Nursery SECTION A. GENERAL INFORMATION A.1. Solicitation Number: AG-0116-S-09-0004 A.2. Quotes Due: October 24, 2008 A.3. Set Aside: Total Small Business A.4. CONTRACTING OFFICE ADDRESS: Department of Agriculture, Forest Service, R-10 Tongass National Forest, Federal Building, 648 Mission St. Acquisition Management, Ketchikan, AK, 99901, UNITED STATES SECTION B. DESCRIPTION B.1. REQUEST FOR QUOTES: This is a combined synopsis/solicitation for a commercial item prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this announcement. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. B.2. SET-ASIDE: This acquisition is totally set-aside for small business concerns, NAICS 561730. The small business size standard for this acquisition is 6.5 million. Submission of a quote constitutes a representation that the contractor is a small business. B.3. ITEM DESCRIPTION: The Forest Service intends to purchase snow removal and sanding services for the Petersburg Supervisor’s Office at 123 Scow Bay Loop Road and Nursery at Nine Mile Mitkof Highway in Petersburg Alaska. Request for Quote includes a base year and two option years. See attached Schedule of Items for pay items. B.4. SCOPE OF WORK: The contractor shall supply all labor, supervision, equipment, supplies, and services necessary to perform snow plowing and sanding services for the Petersburg Supervisors Office and Nursery parking lots and driveways as stated in Performance Work Statement Tasks. B.5. This solicitation contemplates a firm fixed award to one contractor only. Prospective quoters are encouraged to visit the sites to view the project area for themselves. B.6. This solicitation contemplates an award of a base with two option years. Work on the contract would begin November 1, 2008. The option years are subject to FAR 52.217-9 Option to Extend the Term of the Contract. B.7. LOCATIONS: The Petersburg Supervisors Office is at 123 Scow bay Road and the Nursery is located at Nine Mile South Mitkof Highway, Petersburg, Alaska. B.8. DESCRIPTION OF WORK AREAS: Both areas are surfaced with crushed gravel. B.8.1. Petersburg Supervisors Office has two parking lots (see Attached: Drawing of Petersburg Supervisors Office Building). North parking lot is approximately 60’ wide and 100’ long with 40’ driveway entrance. South parking lot is triangular approximately 190’ by 90’ with two 30’ driveways. B.8.2. The Nursery has one parking lot 66’ wide and 70’ long with an approximately 1000’ single lane crushed gravel road. B.9. SNOW REMOVAL REQUIREMENTS: Snow plowing and sanding service shall be the responsibility of the contractor. The Contractor shall exercise good judgment during times of snowfall or storms so that duplication of plowing efforts does not occur. Areas need to be plowed and cleared of snow from the period of November 1st through May 1st depending on snow fall. The contractor will return parking stops displaced by plowing to their original location at the end of the snow season. B.10. PAYMENTS: Payment will be made at the unit prices shown on the Schedule of Items for completed and accepted work. B.11. CONTRACTOR FURNISHED PROPERTY: The Contractor shall supply all labor and supervision as well as supplies and equipment necessary to satisfactorily meet or exceed the performance standards of this contract. B.12. Contractor shall furnish all equipment, snow shovels, and other items needed for the performance of the work in this contract. All equipment must be properly guarded and meet all applicable OSHA standards. If a piece of equipment needs repair and is determined unsafe by the Contracting Officer Representative (COR), the Contractor shall immediately stop using the equipment. B.12.1. The Contractor shall determine and provide personal protective equipment required for the safe performance of work. Protective clothing, equipment, and devices shall, as a minimum, conform to Occupational Safety and Health Administration (OSHA) standards for the products being used. B.13. GOVERNMENT FURNISHED PROPERTY: No government property is being furnished under this contract. B.14. CONTRACTOR RESPONSIBILITY: The Contractor shall provide all labor, materials and equipment necessary for the protection of Government personnel, equipment, furnishings, buildings and building accessories (such as parking lots, sidewalks, etc.) for damage caused by the Contractor’s negligence. Equipment, furnishings, building and building accessories damaged due to work performed by the Contractor, shall be repaired or replaced to their original condition by the Contractor at no additional cost to the Government. SECTION C. PERFORMANCE WORK STATEMENT TASKS C.1. Remove snow from Petersburg Supervisors Office parking lots and driveways. C.1.1. Plow snow Monday thru Friday when snow depth exceeds four inches, or when directed by the COR. C.1.2. Work should be conduct during non-business hours 6PM to 7AM, unless authorized by COR. C.1.3. Plow snow from cross hash areas identified on Attachment #1; Drawing of Petersburg Supervisors Office Building. C.1.4. Pile snow at the west end of the north parking lot and south east end of the south parking lot. C.1.5. Keep garbage dumpsters free of snow and accessible to the sanitation company. C.1.6. Work shall be performed in a manner that will not reduce or restrict the usable space for parking or block pedestrian access. C.2. Sand Petersburg Supervisor’s Office parking lots and driveways. C.2.1. Sand Monday thru Friday after plowing or when directed by the COR. When snow is continuously falling, delay sanding until snowfall has stopped. C.2.2. Work should be conduct during non-business hours 6PM to 7AM, unless authorized by COR. C.2.3. Sand cross hash areas identified on Attached; Drawing of Petersburg Supervisors Office Building. C.3. Remove snow from Nursery parking lots and driveway. C.3.1. Plow snow Monday thru Friday when snow depth exceeds four inches, or when directed by the COR. C.3.2. Keep garbage dumpsters free of snow and accessible to the sanitation company. C.3.3. Work shall be performed in a manner that will not reduce or restrict the usable space for parking or block pedestrian access. SECTION D. QUALITY ASSURANCE AND SURVEILLANCE PLAN: The Quality Assurance and Surveillance Plan outlines the specific service required and promotes continuous improvements in contractor performance. The government expects the Contractor to perform at the acceptable level throughout the life of the contract. SECTION E. PERFORMANCE STANDARDS: E.1. Customer complaints will be substantiated by the COR for validity. Should the COR determine that the complaint is valid, the COR will inform the Contractor orally and in writing and give the Contractor additional time to correct the defect, if additional time is available. The COR will determine how much time is available. E.2. Should the Contractor disagree with the complaint after investigation of the site, the Contractor shall notify the COR. The COR will review the matter to re-examine the validity of the complaint. E.3. Should the threshold of customer complaints or COR acceptance be exceeded and the Contractor is not able to correct the work before the next scheduled cleaning, a deduction of up to twenty-five percent may be taken off the appropriate pay item. E.4. Contractor shall notify the COR within 24-hours of work performance. E.5. Inspection, Payment and Performance Period E.5.1. The Government will visually inspect the services for compliance prior to acceptance and issuing any payments. Inspections will consist of direct observations. E.6. Any Government personnel or visiting public may observe unacceptable services, incomplete work, or required services not performed and they may contact the COR to communicate a complaint. SECTION F. EVALUATION FACTORS: The Government anticipates awarding a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation that will be most advantageous to the Government considering: Experience and past performance Price, including option years SECTION G. The following Federal Acquisition Regulation clauses are incorporated by reference: 52.212-1 Instructions to Offerors-Commercial Items; 52.212-4 Contract Terms and Conditions-Commercial Items; 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items (AUG 2007) the following clauses are checked as included: 52.219-6, 52.219-8, 52.219-28, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-41. Specifically, the following clauses cited are applicable to this solicitation: FAR 52.232-33, Payment by Electronic Funds-Central Contractor Registration and FAR 52.212-3, Offeror Representations and Certifications Commercial Items. H.2. All contractors must be registered in the Central Contractor Registration database (CCR) and include a copy of their representations and certifications (ORCA) with their quote unless they have been completed online. All contractors must be registered in the CCR database and provide evidence of compliance with 52.212-3 (ORCA) to be considered for award. CCR and ORCA may be accessed electronically at https://orca.bpn.gov/. H.3.
