CERTIFY THIS QUOTE IS VALID FOR A PERIOD OF ____________ DAYS PRINTED NAME: ___________________________________ SIGNATURE: _______________________________________ STATEMENT OF WORK JANITORIAL SERVICES Scope of Work: Vendor will provide janitorial services to the Indiana Area Office, Veterinary Services, 5685 Lafayette Road, Suite 400, Indianapolis, Indiana, three times per week. Government Furnished Items: APHIS, VS will provide supplies such as hand soap, toilet paper and trash can liners. Work Requirements: Daily: clean interior and exterior surfaces of entrance glass door and sanitize door metal. Empty all trash receptacles and replace liners. Sanitize receptacles as necessary. Dust all horizontal surfaces (desktops, desk equipment, file cabinets, partition tops, windowsills, ledges, counters, tables, chairs, wall fixtures). Personal desks will be detailed cleaned when cleared. Dust and sanitize phones. Clean break area sink, counter and table to remove spillages. Spot clean interior partition glass as necessary. Vacuum heavy traffic areas. Dust mop hard surface floors. Restock restroom supplies. Clean and sanitize toilets. Clean and sanitize basins. Remove splashes from walls. Mop restroom floors. Weekly: Vacuum wall to wall. Damp mop all hard surface floors, wall to wall. Monthly: Dust high and low including vents, mini-blinds and baseboards. Remove fingerprints/marks from around doors, doorknobs, doorframes and light switches. APHIS, VS has the right to inspect the site and services called for by this agreement, to the extent practicable at all times and places during the term of this agreement. APHIS, VS shall perform inspections and/or observe operations in a manner that will not unduly delay work. Upon award of this contract, all cleaning will be done between 8:00 a.m. and 4:30 p.m.. General Cleaning: When at all possible, the vendor is encouraged to use biobased cleaning supplies to the maximum extent possible without jeopardizing the intended end use or detracting from the overall quality delivered to the end user.
