- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR Subpart 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Offers are being requested, and a written solicitation will not be issued. The solicitation number is OFHEO-09-Q-0001 and is issued as a Request for Quotation (RFQ). The associated North American Industrial Classification System (NAICS) code for this procurement is 621999 with a small business size standard of $10 Million. This requirement is a total small business set-aside, and only qualified offerors may submit quotes. It is anticipated that only one award will result from this solicitation via the issuance of a firm fixed-price commercial contract. All interested contractors shall provide a quotation as set forth below. Description of Services/Scope of Work: The Federal Housing Finance Agency (FHFA) has a requirement for providing a health fair to FHFA employees in order to provide flu shots, various health screenings, and health related information. The complete scope of work is attached to this notice. Period of Performance: The period of performance is for a base period from date of award through October 15, 2009, with four one-year options. Terms and Conditions: This solicitation incorporates the provisions and clauses that are in effect through Federal Acquisition Regulation Circular 2005-27, effective September 17, 2008. Provision FAR 52.212-1, Instructions to Offerors-Commercial Items (Jun 2008) and addendum are incorporated into this solicitation. Addendum to 52.212-1, Modify: (b) Submission of offer(s) is to read as follows: "Submit offers electronically to janice.uthe@ofheo.gov.". Provision 52.212-2, Evaluation-Commercial Items (Jan 1999) is incorporated by reference with the following inserts under (a): "The following factors shall be used to evaluate offers: (a) The variety of health related screenings and wellness exhibits and services available and provided and the ability to at least provide the Minimum Screenings (Blood Pressure, Body Composition, Cholesterol and Glucose, Cardiac Risk Check, Prostate Specific Antigen (PSA)) and flu shots described in the "Background" Section of the attached Scope of Work; (b) The registration process used to secure specific appointment times for employees to receive screenings, tests, consultation, etc.; (c) The company's privacy procedures to ensure the confidentiality of personal health information; d) Past Performance (In the case of an Offeror without a record of past performance or for whom information on past performance is not available, the Offeror will not be evaluated favorably or unfavorably on past performance.); and (e) Price. Technical and past performance, when combined, are more important than price." Provision 52.212-3, Offeror Representations and Certifications-Commercial Items (Jun 2008) is incorporated. Offerors shall either submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (Jun 2008) with its offer or ensure that they are current in Online Representations and Certifications Application (ORCA) at http://orca.bpn.gov. Clauses 52.212-4, Contract Terms and Conditions--Commercial Items (Feb 2007) and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items (Sep 2008), apply to this solicitation. The following additional clauses also apply to this solicitation: FAR 52.217-5, Evaluation of Options (Jul 1990); FAR 52.217-8, Option to Extend Services (Nov 1999) (insert 15 days); FAR 52.217-9, Option to Extend the Term of the Contract (Mar 2000) (first insert: 15 days; second insert: 30 days; third insert: 60 months). Notwithstanding any other provision of this solicitation whether express or incorporated by reference, FHFA is a non-appropriated fund instrumentality and the resultant contract will not be subject to the Contract Disputes Act of 1978. Any claim presented to the Contracting Officer shall be determined by the Contracting Officer, whose determination shall be final and binding. Central Contractor Registration (CCR): Prospective awardee shall be registered in the CCR database prior to award. Solicitation Instructions: FAR 52.212-1, Instructions to Offerors-Commercial Items (Jun 2008) applies to this acquisition as well as addenda described below: A. Quotations shall consist of the following: 1. An e-mail that (a) identifies the solicitation number; (b) the time specified for receipt of offers; (c) the name, address, and telephone number of the offeror; (c) acknowledgement of solicitation and amendments (if applicable); (d) assertion of the offeror's acceptance of the terms and conditions contained in the solicitation; and (e) offeror's Data Universal Numbering System (DUNS) Number. 2. A complete listing of the health related services to be provided and samples of any materials to be distributed to employees on the day of the event. 3. Facility and technical requirements of the site. 4. Past Performance. The offeror shall provide three past performance references, to include: dates of events, name, address, telephone number, and (if available) an e-mail address of the reference point of contact; and a brief description of previous health fair events within the past two years, including a listing of services provided. 5. A description of the registration process used to secure specific appointment times for employees to receive screenings, tests, consultation, etc. 6. An explanation of the company's privacy procedures to ensure the confidentiality of personal health information. 7. Offerors shall provide pricing for up to 150 FHFA employees for each of the two health fair events/locations for the basic year; and pricing for up to 150 FHFA employees for two health fair events/locations for each of the option years, including pricing for flu shots for up to 300 FHFA employees during each of the option years. Prices shall be proposed on a firm fixed-price basis. 8. Representations and Certifications. Offerors shall either submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications-Commercial Items (Jun 2008) or ensure that they are current in Online Representations and Certifications Application (ORCA) at http://orca.bpn.gov/. 9. Questions about the solicitation shall be submitted via e-mail by October 23, 2008, at 2:00 p.m. EDT to Janice Uthe at janice.uthe@ofheo.gov. 10. Quotations are due no later than October 29, 2008, 2:00 P.M. EST. Proposals shall be submitted electronically via e-mail to janice.uthe@ofheo.gov. Late submissions will not be considered. 11.
