Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This procurement is being conducted under Simplified Acquisition procedures pursuant to the authority of FAR Subpart 13.5, Test Program for Certain Commercial Items (10 U.S.C. 2304(g) and 2305 and 41 U.S.C. 253(g) and 253a and 253b), FAR Part 12, Acquisition of Commercial Items (Title VIII of the Federal Acquisition Streamlining Act of 1994 (Public Law 103-355)), FAR 37.4, Nonpersonal Health Care Services Contract (10 U.S.C. 2304 and 41 U.S.C. 253), and FAR Subpart 37.6, Performance-Based Acquisition. This announcement constitutes the only solicitation; therefore, a written solicitation will not be issued. The Billings Area Indian Health Service (IHS) intends to award a fixed-price commercial item contract in response to Request for Quotation (RFQ)-10-09-004-REL. This solicitation is restricted to 100% Small Business concerns. The solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-27. The associated North American Industry Classification System code is 621399 and the small business size standard is $7.0 million. PRICE SCHEDULE - PATIENT NAVIGATOR/NURSE EDUCATOR: Potential offerors shall propose an all inclusive hourly rate for BASE YEAR: 2120 hours @ $________________ per hour = $__________________. OPTION YEAR ONE: 2120 hours @ $______________ per hour = $__________________. PURPOSE OF THE CONTRACT: The purpose of this acquisition is to contract for a Patient Navigator/Nurse Educator for the Northern Cheyenne Service Unit, PHS Indian Health Center, Lame Deer, Montana. WORK SCHEDULE: Full-time, Monday through Friday, 8/9/10 hour shifts or as needed by the Supervisor. The procurement is a nonpersonal health care service contract, as defined in FAR 37.101, under which the Contractor is an independent contractor. The Government may evaluate the quality of professional and administrative services provided, but retains no control over the medical and professional aspects of services rendered (e.g., professional judgments, diagnosis for specific medical treatment). The Contractor shall indemnify the Government for any liability producing acts or omissions by the Contractor, its employees and agents occurring during contract performance. STATEMENT OF WORK: PATIENT NAVIGATION: Reviews all Indian Health Service patients in contract hospitals for monitoring the appropriateness of medical necessity of admissions, continued stays, intensity of service, severity of illness, as well as allocation and utilization of resources for patient care. Works in collaboration with the Contract Health Service (CHS) Department in determining, which referred patients need to be case managed. Is an active member of the Community Health Resources (CHR) committee to report and monitor all patients admitted to private hospitals or needing specialized care in relation to chronic disease. Gathers statics on all patients' case managed providing date to management in demonstrating appropriate utilization of CHS resources and dollars. Coordinates multi-disciplinary patient care conferences for the high risk or medically complex patient. Facilitates interagency implementation of care for patients who may receive or require services from other professional services. Maintains communication and coordination with patients and patient's families to applicable resources for follow-up. Responsible for organization of patient follow-up after discharge as deemed necessary on a case-by-case basis. Maintains communications with referral physician and/or referral hospital. Will provide leadership and coordination of effort to insure effective case management of all referred patients. Special emphasis and attention is to be placed on catastrophic high cost cases, with considerable effort and time being dedicated toward the proper and prudent management of such cases. Promotes the efficient and effective use of Health Care Services, to insure that Service Unit resources are not wasted, and to expedite high quality patient care. The Lead Patient Navigator is a subject matter expert in overseeing and assisting Patient Navigators to help the patient move through the complexities of the health care system, assisting with more timely treatment. The Patient Navigator will function as a "guide" and offer assistance to community members and staff in "navigating" the health care system and accessing related social and financial services to improve chronic disease detection and treatment. A primary function of the navigator is to establish and help maintain communication between patients, their families, physicians, and the health care system while offering additional assistance. Patient navigators facilitate the care of the individuals by performing each of the following duties: (1) Acting as contacts, including, by assisting in the coordination of health care services and provider referrals for individuals who are seeing prevention or early detection services for those who are found to have a symptom, abnormal finding, or diagnosis of, cancer or other chronic disease; (2) Facilitating the involvement of community organizations in assisting individuals who are at risk for or who have cancer or other chronic diseases to receive better access to high-quality health care services; (3) Anticipating, identifying, and helping patients to overcome barriers within the health care system to ensure prompt diagnostic and treatment resolution of abnormal findings of cancer or other chronic disease; (4) Conducting ongoing outreach to health disparity population of the Northern Cheyenne Reservation who are at risk for or who have cancer or other chronic disease to seek preventative care. NURSE EDUCATOR: Responsible for coordinating appropriate trainings for patient navigators and staff on the processes of the Patient Navigator System. Responsible for planning, developing, implementing, and evaluating a comprehensive and cost efficient staff education and competency program for the nursing staff in addition to service unit staff for basic required health related certifications. Responsible for tracking and maintaining records on all staff development programs/trainings. Will coordinate with DON, CMO, Safety Officer for quarterly mock codes. Will provide informal trainings to nursing staff on equipment and skill competencies. Coordinate the Service Unit Development committee on medical trainings and emergency preparedness. Maintains a core group of instructors locally at the service unit to provide required trainings such as ACLS, PALS, BLS. Assures that requirements are maintained for these instructors to remain active. Collaborates with supporting educational training center such as Montana Health Network for proper updates and certifications. Coordinates required trainings such as TNCC and ENPC be offered locally from outside instructors if needed. If budget allows may bring in additional trainings such as NRP, STABLE, burn certification, triage, etc. Approving of trainings will be done through the Director of Nursing. Records of all trainings will be provided to Department Supervisors to be included in staff PMAP. ER NURSE: Performs specialized professional nursing duties in the case of critically ill, severely injured and/or traumatized emergency patients. Assesses all patients presenting for care and triages based on history, physical assessments and chief complaint. Initiates treatments, medications, emergency and resuscitative measures based on appropriates utilization of standing orders and ACLS, PALS, TNCC and ATLS protocols. Administers therapeutic measures as prescribed by the physician such as medications, treatments, intravenous fluids, nasogastric tubes, etc. Records observations, nursing interventions, therapeutic measures, hemodynamic monitoring data, and other relevant to the patients care. Continually monitors and assesses patients' clinical conditions and recognizes, identifies and interprets serious situations and immediately decides and initiates proper intervention independently. Notifies the physician when appropriate. Interprets data obtained from observation, examination, and monitoring and lab values. Discriminates between normal and abnormal finding, and provides appropriate care measure. Provides nursing care to meet the physical, emotional, spiritual and socio-cultural needs of the critically ill patient and family. KNOWLEDGE REQUIRED BY THE POSITION: Certification in BLS, ACLS, PALS, ENPC and TNCC. Advanced professional knowledge of a wide range of nursing concepts, procedures and techniques used to determine quality care and treatment and interpret complex medical information. Working knowledge of various resources and methods used to utilize those resources to meet the needs of the patients. Knowledge of medical terminology, anatomy, and physiology and human disease processes. Skilled at analyzing problems and developing effective solutions for correction, recognizing situations that indicate more significant problems and relates to senior management regarding difficult and complex issues. Exceptional interpersonal skills, good listening abilities, integrity, and leadership qualities. In depth working knowledge of acute hospital discharge planning processes and/or its equivalent of community health related services to plan for continuing care needs. Substantial working knowledge of maintaining an effective reporting and recording system. Knowledge of the operation of health care financing and CHS programs, including pertinent Department of Health and Human Services and IHS policies and regulation. General knowledge of the contract care model of health care service delivery. Knowledge of effective patient/family teaching. Knowledge of patient care services provided. Ability to communicate and interact with persons (staff, health care professionals, community) with different functions, levels of knowledge and requirements to provide optimal health care. Ability to plan and direct the work of others. Knowledge of culture, customs, and health problems of the Native American population, with more specific knowledge of the Northern Cheyenne culture. Knowledge of education curriculum development and teaching strategies in order to develop education and training for a wide range of audience ages, level of knowledge and levels of understanding. Ability to motivate and challenge student and professional nursing staff to pursue and to sustain learning activities which will lead them toward acceptance of responsibility of their learning. Highly developed specialized skills and abilities in making assessments applicable to the care of critically ill and severely injured patients. Ability to serve as an Emergency Room staff member, health team collaborator, patient advocate, and leads and instructs (serves as role model) other ancillary and support personnel. Knowledge of pharmaceutical used in emergency settings, their desired effects, side effects and complications of their use. Ability to manage nursing care for multiple diverse emergency situations (e.g., obstetrics, metabolic crisis, gunshot wound, motor vehicle crash, etc.). Provide skillful assistance to the physician performing such procedures as chest insertion, intubation, central line insertion, etc. Specialized skill in maintaining, operating, and monitoring complex equipment such as mechanical ventilators, cardiac monitors, EKG machines, pacemakers, defibrillators, etc. Specialized knowledge in nursing treatment of such patient problems and conditions such as cardiac dysrhythmia, adult respiratory distress or failure, hypo/hypertensive crisis, and cardio-pulmonary arrest, multiple trauma, including pediatric medical and surgical problems. Ability to prioritize the care of urgent care patients. This position is directly responsible to the Director of Nursing. Consults with the Director of Nursing on nursing staff education objectives and hospital goals. In collaboration they determine long range planning needs and priorities. The contractor functions as a local subject matter expert on the areas described in the Major Duties. Work is accomplished independently. He/She is self directed and has considerable independence and latitude to develop new procedures, methods, or approaches when none exist. The work of the contractor is subject to reviews through performance appraisal activities through the Director of Nursing. GUIDELINES: Guidelines include the federal, state, and local laws, regulations and policies that govern the Indian Health Service as well as IHS circulars and procedure manuals. The contractor uses independent judgment in interpreting and adapting guidelines to unique situations. COMPLEXITY: This position requires an extremely high degree of judgment, creativity, cultural sensitivity and professional expertise in risk management and administration due to the nature of the position and healthcare environment. The work involves relationships with a wide variety of individuals; consultants and healthcare professionals and paraprofessionals as well as patients and their families. There is a high degree of responsibility in recognizing and preventing potential compensatory events and in identifying trends that may result in an adverse outcome. Work requires design, development and implementation of developmental programs. This includes motivating individuals to both attend education programs. Work includes a variety of educational topics. It requires continued effort to stay abreast of changing standards. Providing emergency room nursing care requires a comprehensive and complex range of professional nursing competencies from prompt, decisive assessments of a very wide variety of special patient conditions, to administering special medications, to operating special emergency medical equipment, and reacting to emergency situations without delay or hesitation. SCOPE AND EFFECT: The contractor is responsible for the implementation of an effective Case Management Program and Nurse Education Program. The need to insure that the utilization of facility resources are being appropriately assigned in relation to patient care occurring throughout the facility. All Service Unit Personnel must have awareness of concepts that relate to this as well as the prevention of fraud and abuse during the assignment of health care resources. The effect will have an impact on budget calculations, reimbursement activities, pharmacy services, contract health services, and all other facility healthcare services. In addition, the contractor is responsible for the development of effective education programs that will meet all IHS requirements. This directly affects the quality of patient care and the improved performance of staff. Emergency treatment and care may save lives and outpatient care may decrease future complications and increase the quality of life for the individuals of the Northern Cheyenne Tribe. PERSONAL CONTACTS: Contacts are with patients, their families, other medical facilities, Northern Cheyenne Service Unit, community, Tribal Health Department and with IHS and non-IHS consultants. PURPOSE OF CONTACTS: Contacts with patients are to provide intervention and health care to patients. Contacts with families are to provide information about the care of the patients, often under stressful conditions. Contact with Service Unit staff and other health care providers is to exchange information required to provide patient treatment and care and to arrange for patient follow-up. Contacts with the community are to provide health care teaching or injury prevention strategies. PHYSICAL DEMANDS: Prolonged periods of moving, lifting, as well as manual dexterity, ability to bend, stoop and sometimes physically restrain patients. May require rotating shifts and overtime, may require extended period with little or no sleep. Emotional pressures of dealing with seriously injured, ill or dying patients in the emergency setting induces stress. WORK ENVIRONMENT: Work environment in a medical facility includes exposure to communicable diseases, intoxicated and/or violent patients. May also include the community environment outside of the facility or in the homes of the patients. Personal protective equipment and guidelines to prevent disease transmission are provided. PERIOD OF PERFORMANCE: One Year. GOVERNMENT FURNISHED PROPERTY: The IHS shall provide all necessary equipment and supplies. The Northern Cheyenne Service Unit will be responsible for getting the Contractor access and clearance to all pertinent computer services necessary to carry out his/her duties. REPORTING REQUIREMENTS: The Contractor shall provide the Contracting Officer with evidence that payment of employment taxes has been made 15 days after each quarter. PERFORMANCE-BASED SERVICE DELIVERY SUMMARY: The Patient Navigator/Nurse Educator's performance will be measured based on the following Performance Requirements: (1) Quality of Performance; (2) Timeliness of Assignments; and (3) Customer Service. The Performance Requirements will be measured against the following Government Performance Standards: (1) Provide Patient Navigator/Nurse Educator Services as specified in the Performance Work Statement; (2) Assignments must be completed by established deadlines; and (3) 5 or more customer complaints. The Method of Government Surveillance to determine compliance with the Performance Requirements are as follows: (1) Random sampling by project officer or her designee; (2) Random sampling by project officer or her designee; and (3) Validated Complaints. Deduction Schedule: A 5% deduction shall be assigned to each Performance Requirement. Deductions shall be assessed against individual invoices when services are not performed or do not meet contract requirements. COMPUTER SECURITY: Pursuant to the Federal Information Security Management Act of 2002 (FISMA), the contractor will be required to complete a course in Computer Security Awareness Training (CSAT). This training must be completed annually by all employees including all contractors, volunteers, students, and summer externs. The Contractor will be required to comply with the Federal Information Processing Standards Publication (FIPS PUB) Number 201, "Personal Identity Verification of Federal Employees and Contractors," and the associated Office of Management and Budget (OMB) implementation guidance for personal identity verification for all affected contractor and subcontractor personnel. CONTRACTING OFFICER AUTHORITY: Authority to negotiate changes in the terms, conditions or amounts cited in this contract is reserved for the Contracting Officer. PROJECT OFFICER: The project officer shall be responsible for: (1) Monitoring the Contractors technical progress, including surveillance and assessment of performance and recommending technical changes; (2) Interpreting the Statement of Work; (3) Technical evaluation as required; (4) Technical inspections and acceptance; and (5) Assisting the Contractor in the resolution of technical problems encountered during performance of this contract. INVOICE SUBMISSION AND PAYMENT: The Contractor shall submit its invoice to the Supervisor at the Northern Cheyenne Service Unit, PHS Indian Health Center, P.O. Box 70, Lame Deer, Montana 59043. The Contractor agrees to include the following information on each invoice: (1) Contractor's name, address and telephone number; (2) Contract Number; (3) Invoice number and date; (4) Cost or price; (5) Dates of Service including the number of hours worked; and (6) Remit to address. Payment shall be made by the Billings Area Financial Management Branch, P.O. Box 36600, Billings, Montana 59107. PRO-CHILDREN ACT OF 1994: The Contractor certifies that it will comply with the provisions of Public Law 103-227, Pro-Children Act of 1994, which imposes restrictions on smoking where federally funded children's services are provided. CHILD CARE NATIONAL AGENCY CHECK AND INVESTIGATION (CNACI). A CNACI must be completed for all Indian Health Service (IHS) contractor personnel within the Billings Area. Public Law (P.L.) 101-630, Indian Child Protection and Family Violence Prevention Act and P.L. 101-647, Crime Control Act of 1990, require the IHS to conduct a character and criminal history background investigation on all contractors performing services in IHS facilities. The character and background investigations will be conducted by the Northern Cheyenne Service Unit. PROVISIONS AND CLAUSES: The following provisions and clauses apply to this acquisition. The FAR provision 52.212-1 Instructions to Offerors Commercial Items; and 52.212-3 Offeror Representations and Certifications Commercial Items are incorporated by reference. The provision at 52.212-2 applies to this acquisition and is provided in full text. FAR 52.212-2 EVALUATION-COMMERCIAL ITEMS (JAN 1999): (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Registered Nursing License = 35 POINTS. Potential contractors must submit a copy of State license with the price quote; (2) BLS/ACLS/PALS/ENPC and TNCC Certifications = 35 POINTS. Potential contractors must submit certificates with price quote; and (3) Past Performance = 30 POINTS. The offeror must demonstrate its record of successful performance in past contracts and/or jobs, Government and/or commercial. Each offeror will be evaluated on its performance under existing and prior contracts/jobs. The offeror must list at least three contracts/jobs and include the following information. (1) Name of Government agency/Company; (2) Contract number, if applicable; (3) Dates of Service/Employment; (4) Total contract value/Hourly wage; (5) Description of contract work/Job duties; (6) Contracting Officer/Company Manager and telephone number; (7) Program Manager and telephone number, if applicable. Technical and past performance, when combined, are considered approximately equal to cost or price. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. The following FAR clauses cited in 52.212-5 are applicable to the acquisition: 52.203-6 with Alternate I, 52.219-6, 52.219-8, 52.219-28, 52.222-3, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-39, 52.225-13 and 52.232-33. In addition, the Contractor shall submit a completed copy of FAR 52.212-3 with its offer. All responsible offerors may submit a proposal, which shall be considered by the Agency. Offers shall be submitted to the Billings Area Indian Health Service, 2900 Fourth Avenue North, Room 304, Billings, Montana 59101, no later than 2:00 p.m., on November 6, 2008. The offer must be submitted in a sealed envelope, addressed to this office, showing the time specified for receipt, the solicitation number, and your name and address. Offers will also be accepted by e-mail at Rita.Langager@ihs.gov or by fax at (406) 247-7108. ACCEPTANCE PERIOD: Your proposal must stipulate that it is predicated upon all the terms and conditions of this RFQ. In addition, it must contain a statement to the effect that it is firm for a period of at least 60 days from the date of receipt by the Government.
