(2)two Inspections per year. 1 Job $__________________ SECTION C - DESCRIPTION/SPECIFICATIONS/WORK STATEMENT Contract Service - Boiler Inspections Annual Maintenance Contract for State Boiler Inspections for a one-year period starting after this contract is awarded. Contract may be renewed for up to three additional years. On Each visit one of two types of boiler inspections will be conducted on each boiler. 1.Type A. Internal and External Inspections. 2.Type C, External under Pressure Boiler Inspections. Actual date of each inspection to be prearranged with VA Hospital Listed below. Inspections to be performed during regular working hours of Monday through Friday, 8:00 a.m. to 4:00 p.m. Each Medical Center will issue individual purchase order numbers for all services to be billed to that facility when services have been completed. SECTION H - SPECIAL CONTRACT REQUIREMENTS Contractor should be experienced in boiler/high pressure vessel inspections. All bidders are to furnish a list of recent jobs that are similar in type. List must include address, point of contact and phone number. Contractor to provide proof of liability insurance for this type of work. Awardee will be required to furnish vendor competency statements for staff doing work before inspections are performed. This will include technician's name, training/licensure, years of experience, etc.
