Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation is available on the FBO website. RFQ Number W912PQ-08-T-0032 is being issued as a Request for Quotation. The solicitation documents and incorporated provisions are those in effect through Federal Acquisition Circular 05-26. This procurement is unrestricted. The cost magnitude is estimated to be $100,000 and $200,000. The NAICS code applicable to this RFQ is 621210. The small business size standard is $5,000,000. Under the solicitation, the contractor is required to provide an on site dental practice with the capability to provide examinations to approximately 450 New York Army National Guard Personnel. The place of performance is Camp Smith Training Site, Corltandt Manor, NY 10567. The period of performance is 14-16 Nov., 2008. All technical requirements pertaining to this solicitation are contained in the RFQ Statement of Work (below). The provision at 52.212-1, Instructions to Offerors – Commercial applies to this solicitation. Award will be made to the lowest responsible offeror. Contractors are encouraged to register as an interested vendor for this solicitation and check the FBO website frequently for any changes to the solicitation. Statement of Work: 1. BACKGROUND: Oral health is vital for force health protection. Failure to achieve a standard of oral health could preclude a soldier from deploying. Dental Readiness is a command responsibility. DoD established a uniform dental classification in HA Policy 02-11. DoD policy requires dental screening examinations to be accomplished and documented on 90% of service members. Members of the Army National Guard shall receive an annual dental screening examination to determine their dental classification. The results of that screening examination shall be entered into the DENCLASS database no later than 5 days from the date of examination. The DENCLASS database updates the Army Medical Protection System (MEDPROS). Dental treatment may be required as well, at certain times. 2. OBJECTIVES AND SCOPE: The objective of this State of Work is to conduct and complete the requirement of Dental Readiness on selected Soldiers in the 2/108th IN, 1/69th IN, 1569th TRANS CO, 369th Signal Network Support Detachment, and HQ 53rd Troop Command, utilizing a mobile dental facility. Those requirements include: •Clinical evaluation of the oral cavity by a licensed dentist employing visual clinical exam supported by bitewing dental radiographs. Panographic dental radiographs must represent the Soldier’s current dental condition. Panographs are to be taken if there is no panograph in the record, if an existing panograph is greater than 10 years old, or if the Soldier has had major dental work since last panograph. Also the dentist may order a panograph if deemed clinically necessary to complete a proper dental exam. Radiographs will be taken by dental technicians. Existing adequate panographs that are analog images will be scanned electronically and uploaded into DENCLASS. •Each exam will be entered into the DENCLASS reporting system within 24 hours of examination by ASM Research (ASMR) trained personnel, annotated on the Standard Form (SF) 603 (Health Record – Dental) and on the SF 5570 (Health Questionnaire for Dental Treatment) of the Soldier’s dental record. •Personnel performing the dental screening examination have a moral and ethical obligation to inform the Soldier if they observe or are apprised of any sign or symptom for which the Soldier should obtain further evaluation or care. •A dental screening examination identifies deficiencies but does NOT include the dental treatment required to correct those deficiencies. 3. DEFINITIONS: None 4. GOVERNMENT-FURNISHED PROPERTY AND INFORMATION: Operating space will be provided for Occupational Safety and Health Administration (OSHA) compliant Mobile Dental Practices. 5. CONTRACTOR FURNISHED ITEM: Contractors will provide all items and licensed personnel needed to complete the Dental Readiness requirements, any employee benefits, Vacation days, Paid Holidays and Sick Leave plan. The Mobile Dental Practices must: (1) provide x-ray areas with lead-lined walls and leaded glass for any patient observation points, for x-ray radiation protection, (2) perform digital bite-wing and panographic x-rays, as needed, for exam completeness and to provide digital x-rays for DENCLASS files, (3) have available emergency medical resources consistent with OSHA standards, such as eye-wash station, distilled water tanks, (4) have separate and walled dental operatories to provide confidentiality for patients (5) provide temperature controlled rooms consistent with OSHA standard, (6) have sterilization equipment in separate sterilization area, biohazard waste receptacles, fully functional standard dental chairs, a designated reception area with fully functional office technology including a LAN with redundant computer systems with printing/copying capabilities, and a ventilation system on-board to minimize air-born pathogens. The contractor’s safety program includes equipment training, occupational safety, health, and fire protection guidance to ensure the protection and health of all work personnel. Contractor shall possess an owned fleet of mobile operatories containing a minimum of three (3) vehicles to accommodate multiple events at the same time. Must be ADA compliant with wheel chair access and an onboard wheel chair lift. Government property will also be protected during the period of time such property is under the control or possession of the contractor. 6. SPECIFIC TASKS: Perform a projected 450 dental examinations, to include digital bitewings; with digital panographic x-rays as indicated, in support of mandatory annual dental screenings during 14-16 November 2008. Dental staff will need to work on Friday, 14 November beginning at approximately 1330 to 2030; Saturday, 15 November, 0730 to 1800; and Sunday, 16 November, 0730 to 1200. These tasks will be performed at two separate screening locations at the Camp Smith Training Site, Cortlandt Manor, NY, 10567-5000. Staffing and equipment levels provided by the contractor must be adequate to complete a minimum of 24 dental screening exams per hour at each of the two separate screening locations on Camp Smith. Exam: The oral evaluation shall assess the current state of oral health, risk for future dental disease as well as assessing general health factors that relate to the treatment of the patient. The following measures are required for each periodic oral evaluation: a. Caries risk assessment – Classify Soldiers as low, moderate or high risk for caries as described by the American Dental Association’s 1995 Special Supplement. This assessment will allow the ADCS to identify those at high risk for future disease and appropriately manage their care. b. Periodontal assessment – Periodontal Screening and Recording (PSR), a screening procedure endorsed by the American Dental Association and the American Academy of Periodontology, should be utilized to determine the need for periodontal treatment. c. Tobacco use risk assessment – Classify Soldiers as a smoker, user of smokeless tobacco or as both. d. Oral cancer screening – Perform a thorough oral cancer screening on all beneficiaries as part of all annual examinations. e. Radiographs should be taken only for clinical reasons according to FDA guidelines as determined by the patient’s dentist. Radiographs should be of diagnostic quality, properly identified and dated. A panograph is required to be present in dental record and that it be of adequate quality for diagnostic/identification purposes. There is no time requirement on updating panographic radiographs. However, the panographic radiograph will adequately represent the current oral condition of the Soldier. Each exam will be entered into the DENCLASS reporting system by ASMR trained personnel within 24 hours (including digital x-ray(s) and annotated on the SF 603 (Health Record – Dental) and the SF 5570 (Health Questionnaire for Dental Treatment) of the Soldier’s dental records. The SF 5570 must be signed by the dentist and the Soldier. Setup of contractor’s dental equipment is to be completed by the day prior to event commencement. Setup of contractor’s dental equipment is the sole responsibility of the contractor. Breakdown of the contractor’s dental equipment is the sole responsibility of the contractor. As required, provide timely feedback and work progress/status as to the number of dental examinations completed to the Medical Team Officer in Charge, to include timelines and/or factors that may negatively impact processing 24 Soldier exams per hour. Provide completion status to leader of the Medical Team Officer in Charge as to time requirements to complete jobs to ensure processing 24 Soldier exams per hour. Contractor must provide digital x-rays to the POC at the event at the end of the event. Electronic entry of data into the DENCLASS database is required within 24 hours of completion of the Dental Screening mission to include the upload of any radiographs taken during the event. Contractor shall be licensed on any services rendered and equipment operated during mission. 7. APPLICABLE DOCUMENTS: None 8. PLACES OF PERFORMANCE: Camp Smith Training Site, Cortlandt Manor, NY, 10567-5000. 9. PERIOD OF PERFORMANCE: 14-16 November 2008 (exams only) 10. OTHER CONDITIONS/ SECURITY: a. Safety: The contractor has established a safety and health program in accordance with the Federal Occupational Safety and Health Administration (OSHA) standard. b. Performance work statement is “UNCLASSIFIED” and under HIPAA regulations. Contractor shall notify the contracting officer if requirement exists for secret classification and circumstances requiring secret classification for Readiness impact reporting. c. Contractor’s affiliated dentists must be New York State Licensed to perform exams and treatment. d. Contractor’s affiliated dentists performing examinations must have a chair-side Registered Dental Assistant for soldier charting. 11. ACCEPTANCE PLAN: All work performed, all materials and tasks data to be submitted under this task will be made available to be inspected for compliance with the SOW and accepted by the COR or authorized representative as specified herein. 12. Dental Classification APPENDIX A ESTIMATED WORKLOAD DATA The following data elements will be reported at the end of an event: Date Location List of contract employees (providers, dental technicians and administrative staff) Total number of patients scheduled Total number of patients examined Total number of Soldiers by dental classification: 1, 2, 3 Total number of bitewing x-rays (sets of 4 per patient) taken Total number of panograph x-rays taken Verification of entry into DENTCLASS A by name listing of all Soldiers examined and the dental classification of the Soldier APPENDIX B Department of Defense Oral Health and Readiness Classification System The oral health status of uniformed personnel shall be classified as follows: a. Class 1 (Oral Health): Patients with a current dental examination, who do not require dental treatment or reevaluation. Class 1 patients are worldwide deployable. b. Class 2: Patients with a current dental examination, which require non-urgent dental treatment or reevaluation for oral conditions, which are unlikely to result in dental emergencies within 12 months. Class 2 patients are worldwide deployable. Patients in dental class 2 may exhibit the following: (1) Treatment or follow-up indicated for dental caries or minor defective restorations that can be maintained by the patient. (2) Interim restorations or prosthesis that can be maintained for a 12 month period. This includes teeth that have been restored with permanent restorative materials for which protective cuspal coverage is indicated. (3) Edentulous areas requiring prostheses but not on an immediate basis. (4) Periodontium that: (a) requires oral prophylaxis (b) requires maintenance therapy (c) requires treatment for slight to moderate periodontitis and stable cases of more advanced periodontitis (5) Unerupted, partially erupted, or malapposed teeth that are without historical, clinical or radiographic signs or symptoms of pathosis, but which are recommended for prophylactic removal. (6) Active orthodontic treatment. The provider should consider placing the patient in passive appliances for deployment up to six months. For longer periods of deployment, the provider should consider removing active appliances and placing the patient in passive retention. (7) Temporomandibular disorder patients in remission. The provider anticipates the patient can perform duties while deployed without ongoing care and any medications or appliances required for maintenance will not interfere with duties. c. Class 3: Patients who require urgent or emergent dental treatment. Class 3 patients normally are not considered to be worldwide deployable. (1) Treatment or follow-up indicated for dental caries, symptomatic tooth fracture or defective restorations that cannot be maintained by the patient. (2) Interim restorations or prostheses that cannot be maintained for a 12 month period. (3) Patients requiring treatment for the following periodontal conditions that may result in dental emergencies within the next 12 months. (a) Edentulous areas or teeth requiring immediate prosthodontic treatment for adequate mastication or communication or acceptable esthetics. (b) Active progressive moderate or advanced periodontitis. (c) Periodontontal abscess (d) Progressive mucogingival condition (e) Periodontal manifestations of systemic disease or hormonal disturbances (f) Heavy subgingival calculus (4) Edentulous areas or teeth requiring immediate prosthodontic treatment for adequate mastication or communication or acceptable esthetics. (5) Unerupted, partially erupted or malapposed teeth with historical, clinical or radiographic signs or symptoms of pathosis that are recommended for removal. (6) Chronic oral infections or other pathologic lesions including: (a) Pulpal, periapical or resorptive pathology requiring treatment. (b) Lesions requiring biopsy or awaiting biopsy report. (7) Emergency situations requiring therapy to relieve pain, treat trauma, treat acute oral infections or provide timely follow-up care (e.g., drain or suture removal0 until resolved. (8) Acute Temporomandibular disorders requiring active treatment that may interfere with duties. d. Class 4. Patients who require periodic dental examinations or patients with unknown dental classifications. Class 4 patients normally are not considered to be worldwide deployable.
