This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, HR00011-09-R-0001, is issued as a Request for Proposal (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26 and DFARS Change Notice 2008018. The associated North American Industry Classification System (NAICS) code is 713940 and small business size standard is $6.5 Million. The Defense Advanced Research Projects Agency (DARPA) has a requirement to provide fitness services through a Corporate Membership Program to DARPA civilian and military personnel. Services are to be rendered from date of award through one-year, with four one-year options. Offeror must provide health club facilities in the National Capitol Area (NCR) area, which are within 1 mile distance from 3701 North Fairfax Drive, Arlington, VA. Facilities should provide for the following: Resistive weight training machines, Cardio-fitness equipment, Aerobics classes, Free-weights & related apparatus, shower towel services, and sanitary locker rooms. DARPA’s Corporate Membership Program provides DARPA personnel a monthly subsidy for health club membership dues. That portion of monthly dues not covered by DARPA’s subsidy will be the responsibility of the each individual DARPA employee enrolled in the program and the offeror must allow for payment directly by each employee. The offeror must also provide for the following under this effort: an annual health risk assessment for each new member, a monthly usage report to DARPA summarizing by member by month the number of visits for the twelve preceding months, and liability insurance for all facilities used by DARPA employees. The clauses at FAR 52.212-1, Instructions to Offerors-Commercial Items, FAR 52.212-4, Contract Terms and Conditions-Commercial Items and FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items, apply to this acquisition. The additional clauses cited within the last clause cited are applicable: 52.203-6, 52.219-4, 52.219-8, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.222-39, 52.225-13, and 52.232-33. The DFARs clause at 252.212-7001, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items applies to this acquisition. The additional clauses cited within this clause are applicable: 52.203-3 and 252.232-7003. The following additional FAR and DFARs clauses apply: 52.204-7 Central Contractor Registration, FAR 52.217-9 Option to Extend the Term of the Contract (insert “the period of performance” for the notice in paragraph (a) and 5 years in paragraph (c)), FAR 52.233-3, Protest After Award (AUG 1996) and 252.204-7004 Alternate A. Facsimile and e-mail proposals are not authorized and will not be accepted. The offerors should submit with their proposal the following: (1) Proposal; (2) Cost Proposal; and (3) Completed Representations and Certification in accordance with FAR 52.212-3 Offeror Representations and Certifications--Commercial Items and DFARs 252.212-7000 Offeror Representations and Certifications--Commercial Items., which are available electronically at: https://orca.bpn.gov/; and (4) An affirmation that it has an active registration on the Central Contractor Registration (CCR), which is at the website www.ccr.gov. The technical proposal should include the following: (1) a full description of services, equipment, facilities, and any other benefits provided to health club members; (2) address of facility or facilities available to DARPA personnel; (3) hours of operation for each location; (4) copy of certificate of liability insurance for all facilities available to DARPA personnel; (5) description of polices and health club membership options available to employees leaving DARPA; (6) acknowledgement and agreement of the reporting and health assessment requirements mentioned above; and (7) description of policies for direct payment by DARPA personnel of non-subsidized portion of monthly dues. The cost proposal should include the following: (1) Separate section for each year (base and four option years) with the total monthly rate (including subsidy and employee funded amounts) for the following numbers of DARPA personnel: 1-25, 25-50, 50-75, 75-100, and 100-150; (2) amount (if applicable) of any administrative fees assessed for new members; and (3) description of any other fees associated with health club membership, which can include fees of additional services not normally offered in standard health club membership. The Government reserves the right to make multiple awards as a result of this solicitation if it is in the best interest of the Government. The Government will award multiple Indefinite Deliverable, Indefinite Quanitity purchase order(s) resulting from this solicitation to the responsible offeror(s) whose proposal conforming to this solicitation will be the most advantageous to the Government, price and other factors considered. The following will be used as evaluation criteria for this procurement: 1) Amount and range of services and equipment offered (must at least have minimum requirements specified above, but higher ranking will be given to larger number and quality of services and equipment offered); 2) Convenience of location to DARPA offices, extent of hours of operation; and 3) Price. The factors, i.e., numbers 1 through 3 above, are of equal importance to each other. The exact amount of DARPA’s subsidy per member and amount of members will be determined after receipt of proposals. Monthly rates for each range of membership proposed will be included in the order for each option period. Each awarded option will allow for adjustment of total DARPA personnel covered for that period. Delivery and acceptance of reports is F.O.B. Destination to Defense Advanced Research Projects Agency, 3701 North Fairfax Drive, Arlington Arlington, VA 22203-1714. The Contracting Officer must receive any questions concerning the RFP no later than 10 business days before the response date of this solicitation. An original and 4 copies of the offeror’s proposal shall be received on or before December 4, 2008, 2:00 P.M., local time at Defense Advanced Research Projects Agency, Room 449 3701 North Fairfax Drive, Arlington,VA 22203-1714. The package should be marked with the solicitation number, due date and ATTN: Michael Mutty. The U.S. Postal Service continues to irradiate letters, flats, Express and Priority Mail with stamps for postage and other packages destined to government agencies. Due to potential delays in receiving mail, offerors are encouraged to use alternatives to the mail, such as delivery services, when submitting proposals. Offerors may also hand-deliver their proposals at the mailing address above provided they pre-arrange a time and date prior the closing of the solicitation with Mr. Michael Mutty, michael.mutty@darpa.mil. No emailed or faxed proposals will be accepted.
