Description
NAICS Code: 334111 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 utilizing Simplified Acquisition procedures under FAR Part 13 and as supplemented with additional information included in this notice. Thissolicitation is issued under RFQ-PA-09-00003. This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for three year preventive maintenance on a 7 TB Exagrid Disk-based Data Protection System located in Philadelphia, PA. Term of coverage will be December 6, 2008 through December 5, 2011.STATEMENT OF WORKThe contractor shall furnish the necessary personnel, materials, travel costs to maintain the system described.Location of Equipment: USEPA, 1650 Arch Street, Philadelphia, PA 19103; Period of Performance:December 6, 2008 through December 5, 2011USEPA Project Officer(s): Carol OTormey (215) 814-2677The contractor shall only respond to calls authorized by the Project Officer(s) noted above. The government obligation only extends to properly ordered repair calls. On-site response time shall be at a maximum of four hours or less.All maintenance and repair calls shall be made during normal working hours, 8:00 a.m. - 5:00 p.m., Monday - Friday.In addition, a Project Officer should be present during the maintenance and/or repair call. Repairs should be completed within 5 days of initial call to the contractor.The contractor shall furnish a signed service report when the instrument is being repaired on site. The report should include at a minimum: a. Date of repair; b. Diagnosis of the repair; c. Description of the repair; d. Total cost of repairAll areas not covered under the scope of the maintenance will be billed under separate cover and must be approved by the COR prior to performance. Submit the following information: 1.) Company Name, Address, DUNS #, TIN #, Contact Person; All interested parties must be registered in CCR 2.) Indicate billing cycle (e.g. monthly, quarterly, and annual) Please NOTE: USEPA does not prepay maintenance agreements 3.) Three references for same/similar services performed in the last three years. Include reference name, a point-of-contact, address and phone number, list the contract dates with corresponding awarded dollar amount.
