- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This is issued as a Request for Quote (RFQ) SP4410-09-Q-3004. This RFQ is for the disassemble, transportation and reassemble of a CH-53 helicopter located at Fort Eustis, VA The CH-35 helicopter will be transported from Fort Eustis, VA to Camp Pendleton, CA. Actual addresses are listed in Section B.1.2 of the RFQ. This procurement is a total small business set-aside with the North American Industry Classification Systems (NAICS) Code of 484230. The resulting contract will be a firm-fixed priced contract. All quotes from eligible offerors shall include a completed FAR 52.212-3, Offerors Representations and Certifications - Commercial Items. The following provisions/clauses apply to this RFQ: The following clause under FAR applies: 52.204-7 Central Contractor Registration (APR 2008). IMPORTANT NOTICE: Central Contractor Registration (CCR) applies to all solicitations. Lack of registration in the CCR database will make an offeror/quoter INELIGIBLE FOR AWARD. To be eligible to receive an award from a Department of Defense (DoD) activity, you must be registered in the Central Contractor Registration (CCR) database. To register, access the CCR homepage go to www.ccr.gov or call 1-888-227-2423 regarding questions to the CCR. The clauses and provisions included in this RFQ by reference can be accessed electronically at these addresses: http://www.arnet.gov/far/ and http://farsite.hill.af.mil/. Contract award will be based upon the evaluation factors of price and conformance to the solicitation. Questions regarding this combined synopsis/solicitation RFQ must be submitted in writing via e-mail to sarah.rutkowski@dla.mil. The issue date for this RFQ is 10 Nov 2008, and the closing date is 4:30 p.m. Eastern Time on 25 Nov 2008. Submit complete RFQ packages along with a completed bid schedule and a completed FAR 52.212-3, Offerors Representations and Certifications - Commercial Items by e-mail to sarah.rutkowski@dla.mil or by facsimile to (269)961-4474. All eligible responsible sources may submit a quote which shall be considered for award.
