Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. (ii) This solicitation, FA2517-09-T-5009, is being issued as a Request for Quotation (RFQ). (iii) A statement that the solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-24. (iv) This procurement is a small business set aside. The NAICS code is 562119 Other Waste Collection and the size standard is $12.5 million. (v) Contractor shall utilize the below line item structure when preparing quotes: Line Item 0001 Description: Load/Remove Debris Non-personal services to provide all management, tools, supplies, equipment, labor and applicable licenses and permits necessary to ensure that concrete debris removal and disposal services and diversion methods are performed at Peterson AFB Colorado in a manner that will restore a neat and professional appearance of base areas and will comply with government regulations and commercial standards for concrete debris removal In Accordance With (IAW) the attached Performance Work Statement (PWS) dated 1 December 2008. Prospective contractors should provide quotes based on a price per ton for both concrete and for a mixture of dirt and rock. If fees are involved with a specific dump site, include the prospective fees into your price per ton quote. (vi) The following commercial item is requested in this solicitation: Non personal services for concrete debris removal and disposal at Peterson AFB, CO IAW the attached PWS from 1 Dec 08-30 Nov 09. (vii) Advanced payments are not authorized. To be performed on Peterson AFB, CO from 1 Dec 08 - 30 Nov 09. (viii) The following provisions and clauses apply to this acquisition: The provision at FAR 52.212-1, Instructions to Offerors/Commercial Items. All firms must be registered in Wide Area Workflow (WAWF) at https://wawf.eb.mil at time of quote submittal. DUNS, Cage Code, tax ID, terms and conditions shall be submitted with quote. All firms must be registered in the Central Contractor Registration Database at www.ccr.gov in order to be considered for award. (ix) FAR 52.212-2(a) Evaluation - Commercial Items, applies to this acquisition. Quote will be evaluated based on price. (x) The provision at FAR 52.212-3, Offeror Representations and Certification-Commercial Items, is applicable to this acquisition. All vendors must be registered in Online Representations and certifications (ORCA) (https://orca.bpn.gov)) at the time of Quote submittal. (xi) The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to the acquisition.
